Questions and Answers for 173120 Non-invasive Hemoglobin Testing Machines/Supplies

Q 1: Can you tell me what the Supplier Diversity Program form is listed under 5.0 Submission Requirements?

A 1: The Supplier Diversity Program was established in September, 2010 via Executive Order 524. Massachusetts Executive Order 565 reaffirmed and expanded the Commonwealth’s policy to promote the award of state contracts in a manner that develops and strengthens certified Minority Business Enterprises (MBEs), Women Business Enterprises (WBEs), Minority and Women Nonprofit Organizations (M/WNPOs), Veteran Business Enterprises (VBEs), Service-Disabled Veteran-Owned Business Enterprises (SDVOBEs), Disability-Owned Business Enterprises (DOBEs), and Lesbian, Gay, Bisexual and Transgender Business Enterprises (LGBTBEs).

A commitment to partner with an SDO certified vendor is required in order to award any contract valued at greater than $150,000.00 for the life of the contract. Please review the FAQ document posted with RFR 173120 before completing the SDP form.

We believe that these documents should already be on file with your controller. Please let me know as it will be greatly appreciated. I look forward to speaking with you.

A 2: All of these forms will be required for this bid. The three forms in question would be required before a contract can be executed.
