QUESTIONS AND ANSWERS

RFQ-OSCU-FFDPE2011

1. Where on the pricing matrix should we include the price for HMRU examinations for “Onboard Team Members” as described in SOW Section III.8.2 (p. 53)? There is only a line item for HMRU Initial Examinations (item B.5), which differs from the Onboard exam in a few areas (does not require a 2 view chest x-ray and blood lead/ZPP testing; only one draw is required for both RBC and plasma cholinesterase).

The pricing matrix does not reflect a line item for “Onboard” HMRU exams. Form FD-947 indicates the required components for this exam. All of these components are separately priced in section 14 of the price matrix

2. If we were to submit and be awarded the RFQ but were unable to perform certain items needed (such as Colonoscopy, Pap, Stress Test), would we be responsible for subcontracting these services or would we only perform the items that we could then they would go else where for the other portions?

The price matrix and Statement of Work indicate that certain items/services are not required; however if the services are available, pricing should be provided. All other services are required services and must be available to be considered for award. Subcontracting of services is permitted; however, all subcontractors must clearly be identified in your quote and an explanation must be provided for the oversight conducted over subcontractor performance. All subcontractors are subject to the same terms and conditions regarding security requirements and the prime contractor will be responsible for adherence to contract performance as whole.

3. If we were to subcontract with other departments/clinics to cover such things as Colonoscopy, Pap, Stress Test, Etc., would billing need to be centralized from our location or would you prefer that a bill be sent separately for each service/location?

Please refer to section VI.3.0, Invoicing Requirements. Billing would need to be centralized from the prime contractor.

4. In addition to the basic exam, are we able to charge for the additional services being provided (colonoscopy, pap, Stress Test, immunizations, etc..?

The price for each exam, items B.1 through B.11 each reference a FD form or section of the Statement of Work. These forms and sections specify the required components of each exam. Pricing for exams should be based on these required components. Section 14 of the price matrix includes pricing for individual testing.

5. Will each physical come with an authorization and list of items that are required for this exam?

Please reference the applicable FD form for each exam type. The FD forms are listed as attachments to the solicitation.

6. If we do subcontract out the extras (colonscopy, stress test, etc.)would we need to coordinate the scheduling of these for the same date as the physical exam provided by our office?

No, these can be coordinated for a different timing depending upon availability.

7. If needed, do you grant extensions on getting the RFQ submitted or with the current date the date that everything is due by?

The due date for quotes has been extended. Quotes are currently due by April 15, 2011 at 5PM EDT.

8. Are Nurse Practitioners allowed to perform these physicals or only Board Certified Physicians?

Physician(s) shall conduct medical examinations and laboratory analysis with the exception of GYN and Papanicolau (PAP) portion of exams which may be performed by Nurse Practioners (NP) or Physicians’ Assistants (PA). Physicians must be currently board certified in any of the following: cardiology, emergency medicine, internal medicine, family medicine, or preventive medicine (aerospace, occupational, or general preventive medicine and public health).

9. Would the FBI be available to review the job duties/descriptions with the Providers so they are familiar with some of the terminology and the job duties if needed?

Any questions related to the requirement during the solicitation phase may be directed to the Contracting Officer. After award of the contract, technical representative will be made available to the contractor to answer any technical related questions.

10. Would are clinic be provided a copy of the "survey" being completed by the candidates?

Please review Attachment 13 of the solicitation, which is the Patient Quality Assurance Questionnaire for Vendor Performance.

11. I have had several questions from references for the RFQ-OSCU-FFDPE2011 about returning reference questionnaires directly to us their current contractor. Would we be able to give them your contact information and have them send them directly to you if we are not able to get them to send direct to us?

It is preferred that the past performance questionnaires be included with your quote; however, if necessary, references can send questionnaires directly to the Contracting Officer. The past performance questionnaires should be received by the Contracting Officer by the due date of the solicitation. It is the offeror’s responsibility to ensure references submit information accordingly.

12. In Section III.11.2 and within the pricing Matrix Section B.8 you ask for Hazardous device school exams. Section III states the school is operated in Alabama. Would we need to respond in pricing and technical to this component if we are replying for a location outside of Alabama?

Even though the Hazardous Device School takes place in Alabama, FFD EXAMS MUST BE DONE PRIOR to attending these classes. Yes, you should respond to the pricing and technical aspects of Hazardous Device School Exams for all locations.

13. For male applicants that do not need the GYN Exam, how do we differentiate the price for male and female? Or should the price include the GYN exam for all applicants?

The total price for an applicant exam should only reflect Required components of the exam as indicated by form FD 945 and should exclude any Special Circumstance Component. The GYN exam would be considered a special circumstance component based on the requirements indicated on FD 945. The price for a complete GYN exam is also included as item 14.i. of the price matrix and can be charged separately.

14. Upon review of Form FD-947, both “Plasma cholinesterase” [CPT code 82480] and “RBC cholinesterase” [CPT Code 82482] are included as required components. However, while the price matrix indicates a price for “RBC Cholinesterase x 1 draw” (item 14.dd), no additional line item for the plasma cholinesterase is provided under section 14 or anywhere else. Where should we include the price for the single draw of plasma cholinesterase required by Form FD-947 for the “Onboard” HMRU exams?

The price matrix has been revised for all locations to indicate a new description for pricing for line item 14.dd. Please review amendment 4 which includes revised price matrices.

Line item 14.dd now reflects the following service: RBC Cholinesterase x 1 draw (CPT Code 82482) and Plasma Cholinesterase (CPT Code 82480).

Please provide pricing for these two services combined for line item 14.dd.
