RE: RFQ-OSCU-363108

All Interested Parties:

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

The Federal Bureau of Investigation (FBI) is issuing a Request for Quotation (RFQ), on an unrestricted basis, open to both large and small business concerns, under solicitation number RFQ-OSCU-363108. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-51. The Government anticipates issuing a single contract award for each required location. Each contract will be a fixed price, indefinite-delivery indefinite-quantity type contract. The North American Industry Classification System code is 621111 with a small business size standard of $10.0 million.

REQUIREMENT DESCRIPTION:

The FBI requires the services of board-certified physicians capable of performing complete health examinations for personnel and occupational health services to FBI Field Office Locations in accordance with the attached statement of work. All services must be provided within a close proximity of the Division Headquarters or Resident Agency located in Columbia, SC.

The Government will be required to order and the contractor required to furnish services for at least a minimum of $1,000.00 per contract over the life of each contract. In addition, if ordered, the contractor must furnish any additional services, not to exceed the maximum (non-guaranteed) limit of $2,750,000.00 per contract over the life of each contract.

Ordering Period - The ordering period for this contract is from the effective date of the contract through 60 months.

Period of Performance - Any order issued during the effective period of this contract and not completed within that period shall be completed by the Contractor within the time specified in the order. The Contractor shall not be required to make any deliveries under this contract after 12 months from the expiration of the ordering period.

PROVISIONS: The following provisions apply to this acquisition:

FAR 52.212-1, Instructions to Offerors – Commercial (JUN 2008)

FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items (AUG 2009) FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (JUN 2009) , FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items (JUL 2010)

Additional terms and conditions are contained within the attached solicitation documents.

EVALUATION:

52.212-2, Evaluation – Commercial Items

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

_______ TECHNICAL_ _____________________

________PAST PERFORMANCE________________

_____ PRICE___________________ ____

Technical and Past Performance when combined are significantly more important than price. Price becomes a major factor in award selection when other criteria are substantially equal.

The Government reserves the right to award a contract to an offeror other than the offeror proposing the lowest price when the Government’s evaluation determines a proposal is significantly superior from a non-price standpoint and warrants payment by the Government of a premium.

SUBMISSION OF QUOTES:

Quotes must be submitted in accordance with the FAR provision at 52.212-1, Instructions to Offerors – Commercial Items (June 2008) and the following instructions:

Quotes must be received by the Contracting Officer no later than 12 pm EDT on Thursday May 12, 2011. Quotes shall be submitted in the format indicated in Attachment 28, Instructions to Offerors, to the following address:
