PART 3: Technical Requirements

3.1 Background

The FDA, OBE/ Center for Biologics Evaluation and Research (CBER) and Center for Drug Evaluation and Research (CDER) utilize the Empirica data mining tools to detect safety signals in Vaccine Adverse Event Reporting System (VAERS) and Adverse Event Reporting System (AERS). With the objectives of increasing international pharmacovigilance activities, enhancing the available tools and data to detect safety signals for vaccines, improving the VAERS system, and having access to data regarding products that might be licensed in the future in the U.S., access to the Empirica Vigibase datamining tool would enhance OBE’s current work and mission to monitor the safety of vaccines.

Medical officers in OBE/DE review passive surveillance reports for adverse events reported after vaccination. Currently, medical officers have access to Empirica Signal 7.2 a tool which allows automated data mining of VAERS reports. This tool is one of many methods used to detect possible safety signals. While VAERS receives reports from the US and from foreign countries pertaining to vaccines licensed in the US, it does not capture other international reports for products that are not currently licensed in the US or reported through VAERS. The disproportionality data mining available through this tool will help detect safety signals and generate hypotheses for other studies conducted by OBE.

The World Health Organization (WHO) collaborating center, the Uppsala Monitoring Center (UMC), Sweden was established to develop international signaling systems. UMC collects reports of adverse events from worldwide members of the WHO Drug Monitoring Program. Vigibase is the individual case safety reports (ICSR) database system containing millions of reports, approximately 10% of which are vaccines. The Vigibase data uses the WHO Drug Dictionary and also incorporates a documentation grading score which reflects the quality of the reports.

The access to Vigibase will allow OBE access to increased data which is important for detecting rare adverse events. In addition, many products are first licensed in other countries and thus OBE will be able to review the international passive surveillance reports prior to potential licensure and use of the products in the US.
