Responses to questions for CTS in Akron, OH (RFQ-CTS-026-16)

1. Do you know where I could find out if this RFQ is subject to a subcontracting plan? Yes, a subcontracting plan is a part of the attachments. Please see the SF-1449.

2. Is the Past Performance Questionnaire something that our references fill out? I was reading through it and it appears to be something that someone else fills out about our work as opposed to something we fill out about ourselves? Pass Performance Questionnaire is reference from another business, who work with you in the pass.

3. Do we send those out with our Client Notification letters? I’m not sure the Questionnaire could be completed and returned to us before the RFQ is due to you.

That’s totally up to you, but do know all signed quotes so as to be received by 02:00 p.m., EST on March 24, 2016.

4. We have and plan to continue to subcontract out the mental health pieces of this RFQ. When the RFQ refers to ‘Quoter’ or ‘Offeror’, does that mean us as the primary contractor? My question is hard explained so I hope this makes sense. I guess maybe I should ask it this way – does the subcontractor need to provide everything we do as the primary contractor? They would need to provide a subcontracting plan for review. The content would largely depend on that plan. If the sub is performing in/at Oriana House they would not need the right to use, ADA and other facility information. If the sub is performing elsewhere then all of that information would need to be provided.

5. Do resume’s need to be submitted for non-key staff (i.e. billing clerk, etc.)? Yes - The requirement is that we have resumes for all staff.

6. If the agency we subcontract with to provide mental health services has its own site to see clients, do we have to submit all the documentation required to be submitted by the Offeror (i.e. lease, floor plans for ADA accessibility and group room space, etc.) OR is the Offeror just required to make sure the subcontractor’s comply with the requirements set forth in the SOW? Yes

7. Pages 25-27 of the RFQ ask a series of 17 questions that require a response be submitted. Does the Offeror need to have the subcontractor fill out these 17 questions as well? The quoter is responsible for providing all information related to their proposed performance. They would therefore provide any information as it pertains to sub-contractors as well.

8. We have an active registration in SAM. However, the RFQ mentions ORCA several times. I thought SAM replaced ORCA and ORCA no longer exists. Yes, ORCA is part of SAM

9. The Client and Bank Notification Letters both indicate ‘TDAT Services’. Should this be changed to ‘Community Treatment Services? Yes, you can change it to CTS, the Technical Team will overlook the name era.

10. Is the ‘Service Contract Business Management Questionnaire’ listed on the 2nd page of your letter dated March 9th the same thing as Attachment II – ‘Business Management Questionnaire? Yes

11. Is it correct that a hard copy of the RFQ is not required to be sent with the CD? Submit all proposals on CD/DVD. Page 27 of 42 indicates in bold that two CDs are required. One CD is to contain only the pricing sheets correct? For the other CD that calls for the SF-1449 and Technical Response – are we to assume we need to pull the pricing sheets out of that CD? Submit all proposals on CD/DVD. Please, read SF-1449 Akron, OH (Page 27 of 42)

12. If we hold the current CTS contract that will expire in April, can we use that as one of our three most recent/relevant contracts for the Business Management Questionnaire?

No, because I’m apart of technical & evaluation process.

13. Substance Abuse Family Counseling (2030) Two or more family members (does this include the client coming to this session? This is unclear to me. This is the new service they are requiring us to provide. And does this mean only the family members of one client? The service following that 2040 that we are not to provide says the client attendance is optional. Does that mean in the one we are to do that client must attend or cannot attend? The inmate counts as one of the family members.
