Contractor will at no additional cost to MAFB, be responsible for the performance of all Roof Systems (RS) identified above and all leaks that occur. The period of performance is 5 years from the date of task order award. Period of responsibility begins at task order award or the day after incumbent Contractor’s warranty expires, until the completion of the task order period. Listed in the table above are the expiration dates of the incumbent Contractor’s warranty period.
Prior to any subcontractor performing, the Contractor is required to submit a SF 1413, Statement and Acknowledgement for review by the MAFB Contracting Officer. If there are performance concerns regarding a subcontractor, the Contracting Officer will notify the Contractor.
REQUIREMENTS
1.0 Provide a Total Performance Warranty (TPW): Defined as providing all the work requirements for a square-foot price. Contractor shall issue a TPW not to exceed the performance period of the task order for all building RS identified in this SOW.
2.0 Contractor shall supply all equipment necessary to accomplish the work, including but not limited to ladders, high reach man lifts, and safety equipment. There shall be no Government supplied equipment.
Contractor shall perform work between 7:30 A.M. and 4:30 P.M. daily, Sunday through Saturday, excluding Federal Holidays or military down days, unless specifically authorized in writing by the MAFB Contracting Officer. This does not apply to leak response.
3.0 Contractor Vehicles: Commercial or company vehicles will be allowed access to the base provided company emblems are permanently attached to the sides of the vehicles. Magnetic signs are not acceptable.
4.0 Submittals: The contractor shall complete column 13, "Required Submission Date", on AF Form 66, Submittal Schedule and insert dates for each submittal item not already designated by the Government as having a mandatory due date. Provide the completed form to the Contracting Officer at or prior to the Pre-Performance meeting. The completed form will then be used by the Government for tracking submittal progress. Contractor shall submit two copies of all submittals. Submittal items shall be submitted to the contracting office using AF Form 3000, Material Approval Submittal and shall be marked with a number corresponding to the item number on the AF Form 66, and the page and paragraph number in the SOW requiring the submittal.
5.0 Safety: The Contractor shall comply with all existing Occupational Safety and Health Act (OSHA) standards of safety at all times in the performance of this task order. Hazards to the safe use of the premises due to the Contractor's work and/or equipment shall be suitably marked at all times. Pedestrian and vehicle traffic ways shall be kept clear and unobstructed.
6.0 Power Outages: MAFB will permit power outages and limited utility interruptions in accordance with an approved schedule previously arranged by the Contractor and MAFB facilities personnel. Contractor is responsible for notifying and coordinating all outages with MAFB at least five working days in advance of the intended outage.
7.0 Roofing System Definition: The RS is the weatherproofing assembly and its components that are covered under this TPW including the membrane, insulation, soffit, flashings, termination details, and all sheet metal related details for roofs listed in this SOW, including roof drains, gutters and downspouts. The current condition shall be designated green, amber, or red based on the following remaining life expectancy:
Green 7 + years
Amber 4 - 6 years
Red 3 - 0 years
8.0 Contractor shall collect data by physical inspection and will enter the information in an information database. This web based program shall allow MAFB to view and print AutoCAD® drawings, photographs, and roof reports, as well as update all information within the database. The database shall include information about each roof level of the RS to include composition, type, dimensions, photos, and life expectancy (color coded into green, amber, and red). The Contractor shall assist the BRE as required to maintain the roof management plan.

Employer’s liability coverage of at least $100,000 shall be required, except in States with exclusive or monopolistic funds that do not permit workers’ compensation to be written by private carriers. (See 28.305(c) for treatment of contracts subject to the Defense Base Act.)

(2) Property damage liability insurance shall be required only in special circumstances as determined by the agency.
C.
Automobile liability. The contracting officer shall require automobile liability insurance written on the comprehensive form of policy. The policy shall provide for bodily injury and property damage liability covering the operation of all automobiles used in connection with performing the contract. Policies covering automobiles operated in the United States shall provide coverage of at least $200,000 per person and $500,000 per occurrence for bodily injury and $20,000 per occurrence for property damage. The amount of liability coverage on other policies shall be commensurate with any legal requirements of the locality and sufficient to meet normal and customary claims.
