Amendment 001:
The purpose of this modification is to extend the offers due date from 02/26/2021 to 03/02/2021 and to the following questions. Change to paragraph (vi), see attachment Amendment 001 to RFQ-21-PHX-004 and changes are in red bold.
Questions:
Page 1 Section (v):
1. There is not such a thing as a bifocal progressive and a trifocal progressive. A progressive lens is multifocal. Do you want a price as listed in the RFQ or a single price for progressive lenses? There are progressive bifocal and trifocals. Regular bifocal and trifocal are lined. Just provide prices that are available to your company.
2. Do you want lens priced individually or as pair? Prices as a pair.
3. Transitions is a Brand Name for a Photochromatic tint, do you want a price for generic photochromatic tints, and for Transitions the Name Brand Photochromatic Tint? Provide us what is available to your company.
4. If you do in fact want the pricing for Transitions, which one:
· Transitions Gen 8
· Transitions Gen 7
· Transition Extra Active
· Transitions Drive Wear
· etc.
Provide us what is available to your company.
5. There are different materials for lenses such as CR-39 (plastic), Polycarbonate, Hi-Index. Do you want pricing for all materials or just the standard level which is CR-39? Provide us what is available to your company.
6. Are all lenses to be finished (cut and edged to fit frame style)? Yes
7. Will the lenses be ordered with a frame, or will you ever send a frame to the lab for lenses to be installed? Per the statement of work the contractor will provide frames to the Irene Benn Health Clinic and the patient will choose a frame.
8. What do you mean by clear? Is this simply stating lenses with no tints or anti-reflective coatings? Yes
Page 1 Section (vi):
1. This section states that the procurement will be for a 12 month supply of Contact Lenses. Is this correct, or should it state for optical lenses? Contacts (soft, daily, or gas permeable) are not detailed for pricing is why I ask the question. Our Amendment-001 states “Eye Glass Frames and Prescription Lenses Supplies”, see attachment 20-PHX-0004 Amendment 001.
Page 1 Section (vii):
1. How do you want shipping costs identified. I understand that shipping cost should be FOB Destination Moapa, NV, but do you want shipping built into the lens price, or as a separate line item, or will shipping costs just be pass-through costs invoiced after with shipping bill included as a reference? I believe this is answered on Page 4, shipping cost are to be included in the cost of each item. Yes, shipping cost will be included in the cost of the frames and lenses.
Page 2 Section (viii) Number 3.
1. I have attached a PDF of our Entity Registration with SAMS.GOV, can you confirm this is what you want to be included with the RFQ for documentation to satisfy this requirement?
Yes, copy of the Entity Registration.
Page 2 Section (ix) Letter (b) Options.
This section, states that all items will be added together for a total price for the basic requirement. Can you clarify, please? Do you mean that all the lenses (SV, BF, TF and PAL) will be added together for the basic requirement and then options such as tints, coatings will be added to those? Depending on the patients prescription the patient will have the option to add options. As stated in the BPA Terms and Conditions Paragraph 10. BPA Order a frame and eye glass prescription shall not exceed the cost of $400.00. I've attached a sample pricing spreadsheet we use with all of our Optometrist across the country to help you make sense of what I am trying to ask. If there was a standard list of prices required like the list attached we could ensure all bidders are being compared apples to apples. I hope this makes sense to what I am asking.
I have been through the entire RFQ and believe that answers all my questions needed to turn in our bid for the RFQ. Thank you and I look forward to your response. If you have any questions you can call anytime.
