SCOPE OF WORK

IHS-Hopi Health Care Center (HHCC)

Biohazardous Waste Disposal Service Contract 11 Jan 2016

GENERAL:

The Hopi Health Care Center (HHCC) is located on the Hopi Reservation in Polacca, AZ and serves approximately 7,000 people from the Hopi and Navajo Tribes. It is a 97,000 sq ft building and has been accredited by The Joint Commission. This scope of work (SOW) is for the removal of biohazardous and other miscellaneous wastes for one year plus four optional years. Each year, the Government will have the opportunity to award an additional option year based on the previous years past performance.

SCOPE:

On an annual basis, the HHCC generates approximately 12-15,000 lb/yr of biohazard waste that also includes traces of chemotherapy, pharmaceutical, needles, urine, and all other bio-wastes.

All of the HHCC waste is generated from several departments including short stay (4-bed), OB (4-bed), ED, OPD, radiology, and pharmacy. In this document, the term “waste” will be used to indicate all of these types of wastes combined.

The HHCC uses small, biohazard waste containers (see photos below) that are placed throughout the facility. On a daily basis, the HHCC staff collects and removes the waste from the interior of the facility and it is placed in a fenced-in, covered staging area located on the exterior of the facility.

Contractor shall provide 25, 44-gallon, containers for the waste to be stored in until a pickup is made. Biohazard symbols shall be found on each of the containers. The containers shall have lids and be stackable up to two-high.

Contractor shall perform a pick-up of the waste at least every 14 days and remove it from the property.

Contractor shall provide all labor, materials, and equipment necessary for the proper handling of the waste. All handling, transportation and disposal of such wastes shall be in accordance with RCRA, EPA, DOE and any applicable city, state and Tribal regulations. These wastes may be placed in a sanitary landfill and rendered non-infectious by incineration, steam sterilization, disinfection, or other approved method.

A HHCC scale is available, in the staging area, and the contractor may use this scale to weigh the waste during pick-up. If the contractor chooses to bring their own scale, the scale must show that it has been calibrated within the last 180 days. Waste measured by volume, rather than by weight, is also acceptable.

At the end of each pickup, contractor shall provide to the housekeeping supervisor, the facility manager, or his/her designee, dated written documentation that includes the total weight of waste for pick-up and transport.

Contractor shall also provide 100 laminated placards as shown below. These placards are placed on the biohazard containers to help train staff of proper waste procedures (see photos below).

The containers shall be designed for pharmaceutical use. It is estimated that the pharmacy generates up to 25 gallons of hazardous pharmaceutical waste each year and 20 gallons of non-hazardous pharmaceutical waste on a monthly basis. See attached schedule of forumulary.
