SECTION G – Proposal Submission Instructions

PROPOSAL SUBMISSION INSTRUCTIONS:

Contractors intending to conduct business with the Federal Government must register with the System for Award Management (SAM). SAM is now the primary Government repository, which retains information on Government contractors. You may register via the Internet at www.sam.gov.

All responsible offerors may submit a proposal, which shall be considered by the Agency. Offers shall be submitted to the Billings Area Indian Health Service, 2900 Fourth Avenue North, Room 406, Billings, Montana 59101, no later than 5:00 p.m. MST, on September 6, 2019. The offer must be submitted in a sealed envelope, addressed to this office, showing the time specified for receipt, the solicitation number, and your company's name and address. Offers will also be accepted by e-mail at Kenneth.Nicholson@ihs.gov. Please reference the solicitation number in the subject line.

SECTION H – Evaluation Factors for Award

H.1 – SOURCE SELECTION:

The Government will award a purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers.

1) – TECHNICAL = 50 pts.

Offeror must submit a technical proposal, not to exceed 5 pages, that addresses the following items:

· Descriptive Literature: Information that shows the product’s characteristics or explains its operation for the purpose of evaluation, such as : (1) Design; (2) Materials; (3)Components; and (4) Performance characteristics. Especially if purposing a brand-name or equal product.

· Removal of existing system: Must conform to SOW

· Installation of new system: Must conform to SOW

· In-Service/Training: Minimum of 8 hours and available to 100 staff employees. To be conducted on-site and in-person.

2) – PAST PERFORMANCE = 50 pts.

Offerors must demonstrate their record of successful performance in past contracts and/or jobs. Each offeror will be evaluated on two aspects. The first is the relevancy of the offeror’s past performance to the requirement of this RFQ. The second aspect is the quality of the offeror’s past performance.

Offerors must include past performance of contracts/jobs that are related to this project and include up to (3) three references. Please submit the most recent contract/jobs, within that past 4 years. NOTE: The Government reserves the right to contact any reference provided by the offeror in evaluating the offeror’s past performance. Furthermore, the Government reserves the right to use any information that comes to the attention of the Government in evaluating the offeror.

Technical and past performance, when combined, are approximately equal to cost or price.

3) – COST/PRICE

Price is not assigned an adjectival rating or scored. It will be evaluated to determine whether it is reasonable and if the quotation reflects an understanding of the requirement. This factor will be evaluated utilizing price analysis techniques identified in FAR Part 15.404-1. Analysis will include review of price reasonableness and balanced pricing. Offerors are cautioned that "materially unbalanced" prices and/or unreasonably high or low prices may cause your quotation to be deemed unacceptable and rejected.

H.2 – BASIS FOR AWARD:

Selection of the successful offeror will be made based on the evaluation criteria. The quotations will be evaluated and a contract awarded under the Best Value Continuum approach to permit tradeoffs among cost or price and non-cost factors. This will allow the Government to consider award to other than the lowest priced offeror or other than the highest technically rated offeror who meets or exceeds the technical acceptability standards established in the factors referenced above.

File details come from the government source that posted it.
