B.3 The Contractor shall provide a notice of unplanned interruption of service due to incapacitation of the Contractor to the Contracting Officer.
B.4 Regardless of planned interruptions, the Contractor must have a contingency plan so that linen may be delivered on schedule. The Contractor shall provide an outline of the contingency plan to the Contracting Officer and the Ordering Officer within 10 working days after the contract is awarded.
SECTION: C
C. DESCRIPTION, SPECIFICATION, WORK STATEMENT
C.1 SCOPE:
The Contractor shall pick up; launder soiled/contaminated linens and articles; sort; and then deliver the cleaned articles to the delivery points specified in C.19 below. Transportation of soiled and clean linen used in Bldg. 1400 and at offsite locations shall be in carts provided by the Contractor. Folding specifications are necessary to meet existing storage requirements. The workload estimates stated in the line item schedule and attachments are approximate and in no way are to be considered as a guarantee by the Government as to the amount of work that will be ordered under this contract.
C.2 All work under this agreement shall be in accordance with Center for Disease Control (CDC) guidelines, and Joint Commission of Accreditation of Hospitals Organization (JCAHO) regulations.
C.3 The Contractor shall process all soiled linen articles and return the articles clean within 72 hours from the time picked up from the facilities. Items picked up on Fridays shall be delivered on the following business day.
C.4 All clean, incoming linen shall be listed on, and verified by, the Contractor’s delivery ticket. One copy of each ticket shall remain with the Government. Payment to the Contractor will be for the count of clean linen articles delivered as verified by the Receiving Officer at each place of delivery. All delivery tickets for the Government will be turned into the Technical Assistant in Yokosuka.
C.5 The NMRTC Yokosuka is responsible for monitoring the linen returned to determine any unusual losses. This includes monitoring to ensure the proper departments receive their assigned linen in return after the cleaning process. Linen articles must be returned to the correct delivery point.
C.6 When the Contractor has damaged or lost any Government linen article during transportation and/or during actual laundering, the Contractor shall submit a report of the fact detailing the extent of such damage and/or loss to the Receiving Officer at each place of delivery and the Technical Assistant (TA)* of this contract on the date of delivery. Any loss indicative of pilferage, etc., shall be reported at once to the Receiving Officer and the TA. The TA will also notify the Contracting Officer’s Representative/Assistant Contracting Officer’s Representative (COR/ACOR)* of this contract. The Contractor shall take immediate action to investigate the loss and provide guidance to prevent recurrences.
*The TA is identified further below in paragraph 7 and the COR/ACOR in paragraph 10 under the CONTRACT ADMINISTRATION section.
C.7 The Contractor shall provide all soiled linen bags and mark each bag with the specific locations within the hospital or the clinics using the respective department’s name and room number. In the event linen should become mixed, the Contractor shall attempt to separate and return linen items as required herein by the government.
C.8 Torn and unserviceable linen articles shall be returned directly to the Government Technical
Assistant clean, folded and separated from all other linen articles. All minor rips, tears, open seams of linen found by the Contractor shall be returned to the NMRTC Yokosuka separate from all other laundered and returned linens. Returned damaged linen will be inventoried to account for the loss of government material.
C.9 The Contractor is required to conduct at least one (1) inventory per year for NMRTC Yokosuka at the request of the Government.
C.10 Annual plant inspections will be performed as required by the NMRTC Infection Control Policy by the Infection Control Nurse. When requested by NMRTC Yokosuka, the Contractor shall provide copies of linen facility cultures semiannually to the TA of NMRTC Yokosuka. The Contractor may request in advance a copy of or website link specifying the standards required by the NMRTC Infection Control Policy pertaining to said inspections.
C.11 Routine handling of soiled linen shall meet the following specifications:
A. Soiled linen shall be handled as little as possible and with minimum agitation. Linen bags shall not be opened, segregated, or discarded by the Contractor while at the government’s facility.
B. Linen that is heavily soiled and saturated with blood or body fluids shall be decontaminated and processed by the Contractor to remove stains.
C. Heavily soiled items shall be laundered separately from other linen articles. Items identified for rewash shall be processed accordingly, with no additional charge to the NMRTC Yokosuka for re-laundering. The Government can request stained linen to be processed a second time if it does not meet government standards for hospital use. The Contractor may verify in advance with the TA the specifics of said standards.
D. Linen shall be washed using government approved detergent, in water hotter than 85°C for a minimum of 15 minutes. The Contractor may verify in advance with the TA as to what types of detergents are approved by the Government.
E. Soap, bleach, and germicides shall be used in accordance with each respective manufacturers’ recommendations.
C.12 Any heavily soiled items to be laundered that are contaminated through contact with contagious disease must be considered “CONTAGIOUS.” A person in charge of each post in the hospital and/or the clinics will determine whether an article is “CONTAGIOUS.” “CONTAGIOUS” items shall be placed in water soluble bags and inserted into plastic bags prior to pick-up and clearly labeled as “CONTAGIOUS” laundry. If plastic bags are not available, contaminated laundry will be placed in regular canvas laundry bags specially marked as “CONTAGIOUS.”
A. The Contractor must comply with all local regulations governing the transportation and handling of contaminated and contagious linen articles. The Contractor shall institute change in procedures and provide special equipment to control the spread of infectious disease.
B. The Contractor must have facilities available where soiled and contaminated laundry can be received, sorted and stored separately from clean-processed linen articles and no interchange of personnel exits.
C. According to Japanese law, if linen is contaminated with infectious disease pathogens, the Contractor may only collect linen after disinfected at the hospital or clinic. The Contractor is allowed by Japanese law to collect materials (linen, scrub, sheet, towel, pillow case, pajamas, etc.) other than those contaminated by diseases listed under paragraph C.24, “INFECTIOUS DISEASES,” further below.
C.13 All clean linen shall be stored and transported in carts used exclusively for clean linen or carts that have been decontaminated with a tubercular disinfectant after being used for soiled laundry.
C.14 The Contractor shall provide a written Infection Control policy to the TA within 10 working days after the contract effective date. The Contractor shall ensure that infection control and sanitation standards are upheld in the laundry area. The Government representatives will have the right to hold periodic inspections in the Contractor’s facility.
The Infection Control policy shall address the following:
A. Inspection standards must specify areas to be inspected on either a scheduled or unscheduled basis and identify the individuals who will conduct the inspection.
B. Standards must include a method for identifying deficiencies in quality performance before the level of performance becomes unacceptable.
C. A file of all inspections conducted by the Contractor and the corrective action taken. This file shall be made available to the Government during the term of the contract. The Government Representatives (e.g., representatives of the Preventive Medicine Dept. in NMRTC Yokosuka) have the right to conduct unannounced inspections of laundry facilities and associated records.
C.15 The services rendered under this agreement are subject to Government inspection, during both the
Contractor’s operations and after completion of all tasks. The Government Quality Assurance Surveillance Program is not a substitute for Quality Control by the Contractor. All costs associated with rework are the responsibility of the Contractor. The Government reserves the right to choose the inspection method to be used in implementing the Quality Assurance Program and to verify the inspection methods during the work without notice to the Contractor. The Government also reserves the right to have the Contractor redirect linen to other delivery points in the case of shortages or unbalanced clean linen inventory amounts. In such case, the TA will communicate the government’s linen delivery request to the Contractor in advance.
C.16 The TA and the Contractor are responsible for monitoring the progression and overall technical management of performance hereunder and should be contacted regarding questions or problems of a technical nature.
C.17 Employee training is required. All laundry personnel must be specifically trained in their role in infection control by the Contractor. New employees need intensive initial training. All employees need regular refresher training to include emerging hospital laundry techniques and methods.
Documentation of training is required and will be checked during site visits. A syllabus outlining training must be maintained by the Contractor to ensure coverage of the proper subject matter.
C.18 The Contractor shall follow the specifications for the folding of laundry for NMRTC Yokosuka, VET Yokosuka, BHC Atsugi and BHA Camp Fuji.
C.19 Terminology and definitions for flat-work finished and rough finished laundry are as follows:
A. Flat-work finished laundry shall be washed, dried, pressed, folded, and sorted, according to item and size. Flat-work finished includes: doctor coats, pillowcases, scrub coats, pants, flat sheets, hand towels, surgical wraps, table cloths, and napkins.
B. Rough-finished laundry shall be washed, dried, and folded according to item and size. Rough-finished items include: laundry bags, infant apparel, pajama coats and pants, bath towels, bed spreads, curtains, and other miscellaneous items.
C.20 Assigned linen is allocated to the departmental delivery point. This linen shall be labeled with the delivery point room number, department name, “USNMRTC”, and date of labeling. All scrubs (blouses and pants), lab coats, doctor’s coats and pajamas shall be labeled accordingly by the Contractor. The Contractor will also label new linen received from the TA periodically throughout the year for accountability and to verify that proper linen articles return after the cleaning and delivery process.
C.21 All Contractor-supplied linen bags shall be supplied and maintained in good working order by the
Contractor. Linen bags must be labeled or tagged with the delivery point room number, department name, and “USNMRTC”. Handwritten numbers and letters on the outside of linen bags will not be accepted. Labels will be made of a durable material (plastic or metal) that is easy to read by the generating location. Labels shall be attached to the bags with a strong metal ring or rope and replaced if lost, damaged, or unreadable.
C.22 Soiled linen bags shall be held in place for use by a linen bag hamper. Linen hampers shall be supplied and maintained in good working order by the Contractor. Hampers shall be used in all soiled linen locations and delivery points. Hampers must hold the linen bag open while keeping the bag off of the floor. Linen hampers shall have a lid or cover to keep odor contained and soiled linen out of view. Each hamper shall be labeled with the words “SOILED LINEN” on the top center of the lid/cover.
C.23 Delivery Point: The Contractor shall pick up soiled linen at the locations below and deliver laundered linen back to the same locations at each of the following points by identifying/labeling each place of delivery on the linen item itself or the linen bag/or cart:

QUALITY ASSURANCE SURVEILLANCE
1. Quality Assurance Surveillance Plan (QASP) for Laundry Service
a. Objective: This QASP has been developed by the Government to provide effective and systematic surveillance for the Laundry Service for NMRTC Yokosuka contract. The QASP provides the methodology by which a Contractor’s performance will be monitored to determine compliance with standard performance objectives. The plan is designed so that surveillance is limited to that which is necessary to verify that the Contractor is performing the services satisfactorily and relates directly to performance objectives of the Performance Work Statement (PWS).
b. Method of Surveillance: Surveillance of Contractor performance in this method will be used by the Government to determine whether the Contractor is effectively and efficiently complying with all terms and conditions of the contract. The following surveillance methods shall be used:
Performance Objective Measure Standard Surveillance Quality of Service Whether or not the
Contractor has completed all the requirements in a timely manner
All work shall be performed in strict compliance with applicable contract requirements.
100% Inspection by the Technical Assistant (TA)
Regulatory Compliance Whether or not the Contractor is in compliance with all applicable federal, state and local regulations, laws and ordinances
All work will be performed in strict compliance with applicable federal, state and local regulations, laws and ordinances.
100% Inspection by the
TA
Timeliness and Accuracy of Invoices
Whether or not the invoices are submitted in a timely manner and are accurate
Invoice will be submitted in an accurate and timely manner (within 10 working days of the following monh) 100% of the time. Any required resubmissions of invoices are corrected within three
(3) working days.
100% Inspection by the
TA
c. Evaluation and Rating: The COR/ACOR will report the Contractor’s performance in each operation and rate the performance by using the form of “Contractor’s Performance Evaluation Sheet” as shown in Attachment (2) of this contract. In case any inconsistency or deficiency is found, the COR/ACOR will record it on the form and attach any related documents to the form.
The COR/ACOR will submit the Contractor’s Performance Evaluation Sheet to the Contracting Officer on an annual basis. The Government will rate the Contractor’s performance using the following evaluation ratings:
EXCEPTIONAL (E): The services performed by the Contractor are considered to have exceeded the performance standards described above and demonstrate a good probability of success without any problems during the remaining period of the contract.
VERY GOOD (VG): The services performed by the Contractor are considered to have satisfied the performance standards described above and demonstrate a good probability of success during the remaining period of the contract. Even if the Contractor had minor problems on its performance, the Contractor has demonstrated that it could correct such problems without any damage to the Government.
SATISFACTORY (S): The Contractor’s performance is considered to have substantially met the stated performance standards described above and demonstrates a good probability of success during the remaining period of the contract. Even if the Contractor had minor problems on its performance, the Contractor has demonstrated that it could correct such problems with minimal damage to the Government. Although some minor inconsistency or weakness exists, it can be readily corrected.
MARGINAL (M): The Contractor’s performance is considered to have partially failed meeting the stated performance standards, and the response to correct the deficiency was marginal. That is, a substantial number of the Contractor’s record responses “good,” but a deficiency or weakness exists, and it/they may be susceptible or correction through the exchange of information. There is marginal probability of success during the remaining period of the contract.
UNSATISFACTORY (U): The Contractor failed meeting one or more of the stated performance standards, and failure was considered “major.” Even if the failure was minor, the Contractor did not or could not correct the deficiency properly and/or if adversely affected the Government operation. In such a case, probability to success during the remaining period of the contract is minimal unless a proper corrective action is made.
In case the Contractor’s performance is rated either “Marginal” or “Unsatisfactory”, the Contractor shall take corrective action(s) to all deficiencies identified 30 days before the next term begins.
d. Incentive/Decrement: For favorable evaluation record(s) of the Contractor’s performance, the Contracting Officer may consider reducing frequency of surveillance, which is currently 100%, for the remaining contract period including option years. When the Contractor performs "nonconforming services" (e.g., services rated "Marginal" or "Unsatisfactory"), such decremental records/reports may unfavorably affect future contract award. The Government may seek an equitable adjustment in accordance with the clause, "FAR 52.212-4(a), Inspection/Acceptance.
e. Record of Evaluation: The Contracting Officer will maintain a complete QASP file. All records will be retained and considered when deciding if it is in the best interest of the Government to exercise an option to extend the term of the contract or enter into a new contract.
2. Inspection: Services performed by the Contractor will be inspected by the Receiving Officer at the place of pick-up and delivery of the linen articles.
3. Acceptance: Services performed by the Contractor will be made by the Receiving Officer of each location by signing the laundry order sheets.
