Title: Estimating medical expenditure before and after type 2 diabetes diagnosis by levels of plasma glucose and A1c before diabetes onset Period of Performance: 8/1/2020 to 1/31/2022
SECTION 1 – BACKGROUND
Diabetes costs Americans more than any other disease. It was the most expensive condition among the 155 most common diseases in the country in 2013. Care for people with diabetes was responsible for 1 in 4 US healthcare dollars in 2017. The annual medical expenditure is $16,750 per person with diagnosed diabetes or 2.3 times as much as those without the disease. The medical cost associated with diabetes has been increasing over time. From 2007 to 2017, this cost has increased by 73% from $137 billion to $237 billion. It is expected that the medical cost associated with diabetes will continue to increase in the foreseeable future.
Various types of diabetes prevention programs have been developed and implemented during the last decade. Some programs focus on the whole population-level (e.g., sugar/beverage taxes) while others are exclusively targeting high-risk populations (e.g., lifestyle intervention). To understand the value of different prevention programs, one needs to know the economic benefit of preventing or delaying the onset of diabetes by implementing interventions using both population-level and high-risk population approaches. Such benefits could be calculated as the difference in increase in medical expenditures when a person moves from a non-diabetes status (or pre-diabetes) to a diabetes status. More specifically, one needs to know the medical costs of both the pre-diabetes stage and after the onset of diabetes by a person’s baseline risk level of developing type 2 diabetes. These estimates also provide essential information needed by the CDC’s cost-effectivness diabetes simulation model for assessing the long-term cost-effectiveness of various interventions for preventing/managing type 2 diabetes.
Previously, researchers assessed the incremental medical expenditures for adults with newly diagnosed type 2 diabetes, for up to 10 years before and after diabetes diagnosis. This study demonstrated that people diagnosed with type 2 diabetes had higher medical expenditures compared with their counterparts, not only after diagnosis but also up to 10 years prior to diagnosis. Due to a lack of glucose measurements, the authors were not able to differentiate expenditure estimates by level of risk for developing type 2 diabetes. However, persons at different levels of glucose and/or A1c before the onset of diabetes may have different trajectories in health care expenditures before and after diabetes diagnosis. Estimating these costs is pivotal to improve the accuracy of financial benefits saved and the cost-effectiveness of type 2 diabetes prevention programs targeting populations at varying levels of diabetes risk.
SECTION 2 – PURPOSE
The purpose of the proposed study is to estimate the medical expenditure in years before and years after a person is diagnosed with type 2 diabetes by her/his baseline glucose and/or A1c levels. Longitudinal follow-up data from Optum de-identified Normative Health Information (dNHI) database are the most appropriate data for this analysis as such data contain both health care utilization or cost and glucose measurements for their populations. Glucose measurements from lab tests are needed for the study to establish the baseline risk of type 2 diabetes. Using longitudinal data and grouping persons without diagnosed diabetes by their baseline risk levels of developing type 2 diabetes can overcome the two potential estimate biases available from the current literature by allowing (1) persons who do or do not develop diabetes and persons with different risk levels of developing type 2 diabetes who have different trajectories in medical expenditures. At least three cohorts of persons without diagnosed diabetes, i.e., those at the low, intermediate, and high-risk level of developing type 2 diabetes shall be created and followed. The follow-up time shall be at least ten years (i.e., 5 years prior and 5 years after the diagnosis of type 2 diabetes) to get a reliable estimate.
The specific objectives are:
(1) To create 3 cohorts of persons who have developed type 2 diabetes by their risk levels at least 5 years prior to the diagnosis of diabetes. The low risk group could be defined as those with an annual progression rate of below 3%, the intermediate risk group with an annual progression rate of 3 to 5% and the high-risk group with an annual progression rate of greater than 5%.
(2) To create a comparison cohort of persons who have not developed type 2 diabetes during the corresponding study period.
(3) For each cohort:
a. To estimate the annual utilization and expenditure associated with inpatient services at least 5 years before and after the diagnosis of type 2 diabetes
b. To estimate the annual utilization and expenditure associated with outpatient services at least 5 years before and after the diagnosis of type 2 diabetes
c. To estimate the annual utilization and expenditure associated with medications at least 5 years before and after the diagnosis of type 2 diabetes
d. To estimate the annual utilization and expenditure associated with other medical care services at least 5 years before and after the diagnosis of type 2 diabetes
e. To estimate the total annual medical expenditures at least 5 years before and after the diagnosis of type 2 diabetes
(4) To estimate the difference in cumulative total medical expenditures before and after the diagnosis of type 2 diabetes for each risk group
(5) To estimate the difference in cumulative medical expenditures between those who have developed diabetes (case group) and these who have not developed diabetes (comparison group) before and after the diagnosis of type 2 diabetes by risk group
(6) To perform the above analysis using at least two age groups, i.e., 18 to 65 years and 65 years and older
SECTION 3 – SCOPE OF WORK
The Contractor shall furnish all necessary labor, facilities, supplies, and equipment to:
(1) Obtain the longitudinal data needed for the analysis
(2) Create 3 case cohorts and one comparison cohort with a sample size of at least 3,000 for each cohort for the analysis
(3) Estimate the annual utilization and expenditure associated with each cohort by service type at least 5 years before and after the diagnosis of type 2 diabetes
(4) Estimate the difference in cumulative total medical expenditures before and after the diagnosis of type 2 diabetes for each risk group
(5) Estimate the difference in cumulative medical expenditures between each case group and the comparison group before and after the diagnosis of type 2 diabetes
(6) Write one manuscript based on the findings of the study that is ready for CDC clearance
(7) Submit the CDC cleared manuscript to a peer-refereed journal
(8) Revise the manuscript until it is accepted for publication.
SECTION 4 – TASKS TO BE PERFORMED
The Contractor shall perform the following tasks within the time period specified:
1. The Contractor shall provide bi-monthly progress reports to include the current status of work completed, the projection for the following month, assessment of milestones and strategies, and impact on meeting completion deadlines; barriers/issues to be resolved; and contractor’s internal efforts to maintain and improve quality of work. This will consist of a narrative report and an attached expenditure report.
