The Federal Bureau of Investigation (FBI) Procurement Section is issuing this Request for
Quotation (RFQ) to solicit the authorized manufacturer and resellers of Vision Engineering, Inc.
to establish a one-time, firm-fixed-price contract to acquire the following brand name products:
Four (4) quantity Lynx EVO ergonomic wide field stereo microscope heads; Four (4) quantity Lynx Evo head 10:1 high performance zooms; Four (4) quantity Lynx EVO ring lights; Four (4) quantity .62x objectives; Four (4) quantity power units; Four (4) quantity
Lynx Evo multi-axis stands, counterbalanced; Four (4) quantity Multi-Axis adjustable stands for larger components base; Four (4) quantity Multi-Axis stand focus assemblies;
and Four (4) quantity Smart Cams for Lynx Evo; including shipping.
Contractors interested in competing for this effort may participate by submitting quotes in accordance with the procedures set forth in this RFQ.

Brand Name only: The Buyer is allowing Sellers and Manufacturer to submit bids for the items, provided the items meet all of the salient physical, functional, or performance characteristics specified by this solicitation. Sellers or Manufacturer MUST enter exactly what they are bidding (including make, model and description) into the blank description field in order for the bid to be considered. The Buyer will evaluate items on the basis of information furnished by the Seller or identified in the bid and reasonably available to the Buyer. The Buyer is not responsible for locating or obtaining any information not identified in the Bid.

General Information
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. No partial quotes will be considered.
No partial awards will be made.
The North American Industrial Classification System (NAICS) code for this requirement is
333314 – Optical Instrument and Lens Manufacturing; the size standard is 500 employees. The resultant contract is intended to be awarded as a one-time, firm-fixed-price contract. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2020-
04. This requirement will be awarded on an all or none basis as a commercial item under the procedures at FAR Parts 12 and 13.
Requested Products
The Federal Bureau of Investigation (FBI) requests open-market pricing for the Vision Engineering, Inc.
brand-name products listed below. If vendor is providing a product, the item must contain all technical specifications included. A Brand Name Justification will be made available upon request. If these descriptive(s) are not included in the submitted quote, the quote shall be considered “unresponsive.”

Quotation Requirements
• New products only shall be accepted for this procurement.
• Quote shall be firm-fixed-price.
• Offers shall be valid for 60 calendar days after submission.
• All vendors shall have an Active DUNS number in SAM.gov.
Please include in your quote: pricing and product description for each line item listed above. Quote may be sent via your company's standard quotation form but must include the following information:
Tax ID #, DUNS#, Point of Contact with email and telephone information.
Submission of Quotations
Quotations/offers and any accompanying information shall be submitted via email to the attention of Naporshia Jackson, Contracting Officer, at nljackson@fbi.gov, no later than Friday, September 17, 2021, at 12:00pm CST. Quotations/offers sent via other means (i.e. telephone call, facsimile transmission, etc.) will not be considered.

5.1 Access to Federal Bureau of Investigation (FBI) Locations:
e-QIP - Contract Security Requirements from Acquisition Security Unit (ASU), Procurement
Section Directive (PSD): 09-22.18
Performance under this contract may require access to FBI locations to provide some service, product, or perform some other official function of interest to the FBI. Requirements, as identified below, to include approval by the FBI's Security Division, must be satisfied prior to access. Contractors who will require escorted access, to include short-term, intermittent, or infrequent access, to an FBI facility must complete an "Access of Non-FBI Personnel to FBI Facilities, Background Data Information Form," (FD 816), a
"Privacy Act of 1974 Acknowledgment Form" (FD 484) and two Fingerprint Cards (FD 258).
Completed forms should be provided to the assigned Contracting Officer's Technical Representative
(COTR) at least 10 days prior to required access. Individuals requiring unescorted access to an FBI facility must complete the Standard Form 86 (SF-86), Questionnaire for National Security Positions, using the Office of Personnel Management’s Electronic Questionnaires for Investigations Processing (e-
QIP) and provide two Fingerprint Cards (FD 258). e-QIP is a secure website that can be accessed from any computer system which has an Internet connection. Only the signed release forms and FD 258 will need to be mailed to the identified Chief Security Officer, the SF-86 itself will be transmitted to the FBI electronically. To complete the SF-86 using e-QIP, the individual requiring unescorted access to the FBI facility must contact (insert Chief Security Officer, Division, and telephone number) in order to be initiated into e-QIP. Once this action has been accomplished, the individual should be able to access e-
QIP at the following link in order to initiate and complete the electronic process: http://www.opm.gov/e-qip/browser-check.asp.
Thoroughly read and follow the instructions for completing the SF-86. NOTE: To fully address suitability/security issues, the FBI requires individuals to provide responses to questions on the SF-86 for the last ten years. Failure to complete the application as instructed may lead to significant delays in processing the required investigation and approval for unescorted access. Upon logging onto e-QIP, there will be a prompt to answer three “Golden” security questions to establish the user account. After completing the electronic SF-86, please print and sign the (1) Certification Form (CER) - Certify
Completeness and Accuracy of your Investigation Request; (2) Medical Release Form (MEL) -https://www.acguisition.gov/far/ http://www.opm.gov/e-qip/browser-check.asp http://www.opm.gov/e-qip/browser-check.asp

Authorization for Release of Medical Information; and (3) Release Form (REL) - Authorization for
Release of Information. In addition to these SF 86 release forms, the completion of a Non-Personnel
Consent to Release Information (FD-979a), the United States Department of Justice Disclosure and
Authorization Pertaining to Consumer Reports (DOJ 555) are required. Annotation of the assigned e-QIP
Investigation Request Number on the upper right corner of each document transmitted to the identified
Chief Security Officer is required for coordination with the electronic transmission and to facilitate the investigative process.
The e-QIP Investigation Request Number, automatically generated by e-QIP, is located on both the header and footer of the signature forms. These release forms (five total) and FD 258 should be mailed via Federal Express or UPS Express mail directly to the following address: (insert name and address of
Chief Security Officer). The use of regular U.S. mail channels may cause significant delays in processing the unescorted access request. Upon completion of processing the facility access request, the individual will be required to execute a non-disclosure agreement suitable for their approved access.
Corporate Representation Regarding Felony Conviction Under Any Federal Law or Unpaid
Delinquent Tax Liability - (DEVIATION 2015-02) (March 2015)
(a) None of the funds made available by the Department's current Appropriations Act may be used to enter into a contract, memorandum of understanding, or cooperative agreement with a corporation –
(1) convicted of a felony criminal violation under any Federal law within the preceding 24 months, where the awarding agency is aware of the conviction, unless an agency has considered suspension or debarment of the corporation and made a determination that this further action is not necessary to protect the interests of the Government, or
(2) that has any unpaid Federal tax liability that has been assessed, for which all judicial and administrative remedies have been exhausted or have lapsed, and that is not being paid in a timely manner pursuant to an agreement with the authority responsible for collecting the tax liability, where the awarding agency is aware of the unpaid tax liability, unless an agency has considered suspension or debarment of thecorporation and made a determination that this further action is not necessary to protect the interests of theGovernment.
(b) By submitting a response to this solicitation, the offeror represents that, as of the date of this offer –
(1) the offeror is not a corporation convicted of a felony criminal violation under any Federal or State law within the preceding 24 months; and,
(2) the offeror is not a corporation that has any unpaid Federal or State tax liability that has been assessed, for which all judicial and administrative remedies have been exhausted or have lapsed, and that is not being paid in a timely manner pursuant to an agreement with the authority responsible for collecting the tax liability.
Contractor Internal Confidentiality Agreements or Statements Prohibiting or
Restricting Reporting of Waste, Fraud, and Abuse - Solicitation - (DEVIATION
2015-02) (March 2015)
None of the funds appropriated to the Department under its current Appropriations Act may be used to enter into a contract, grant, or cooperative agreement with an entity that requires employees or contractors of such entity that requires employees or contractors of such entity seeking to report fraud, waste, and
RFQ # 15F06721Q0000426 Version 1.0 Page 5 abuse to sign internal confidentiality agreements or statements prohibiting or otherwise restricting such employees or contractors from lawfully reporting such waste, fraud, or abuse to a designated investigative or law enforcement representative of a Federal department or agency authorized to receive such information. By submitting a response to this solicitation, the contractor certifies that it does not require employees or contractors of the contractor seeking to report fraud, waste, and abuse to sign internal confidentiality agreements or statements prohibiting or otherwise restricting such employees or contractors from lawfully reporting waste, fraud, and abuse to a designated investigative or law enforcement representative of a Federal department or agency authorized to receive such information.
System for Award Management (SAM) Requirement
Please ensure your firm is registered in System for Award Management (SAM) – All vendors must be registered in SAM, effective July 29, 2012, to receive government contracts. The FBI utilizes a financial system that has a direct interface with the SAM. If any current FBI vendor data conflicts with SAM Data, the information contained in SAM may override our current information. Please ensure that your company's SAM information is updated and accurate. This includes: TIN, EFT, DUNs, addresses and contact information. The EFT banking information on file in SAM will be what the FBI uses to process payment to your organization.
Evaluation Factors
Award shall be made to the offeror whose quotation, conforming to this solicitation, is determined to be Lowest-Price Technically Acceptable.
