& ANTIBIOTIC SUSEPTIBILITY TESTING (AST)
1. The 60th Contracting Squadron at Travis Air Force Base intends to award a Firm Fixed Price contract for Automated Microbiology Identification and Antibiotic Susceptibility Testing for the Medical Treatment Facility at the David Grant Medical Center at Travis Air Force Base, California.
2. General Information:

3. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.
Additionally, the Government will utilize simplified procedures in accordance with FAR Part 13.
4. T A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-03 effective 16 Feb 2021, Defense Federal Acquisition Regulation Change 1/15/2021 effective 15 January 2021, and Air Force Acquisition Circular (AFAC) 2019-1001 effective 01 October 2019.
5. The North American Industry Classification System (NAICS) code for this solicitation is 325413 – In-Vitro Diagnostic Substance Manufacturing. Small Business Size Standard is 1,250 employees.

7. The provision at FAR 52.212-1, Instructions to Offerors - Commercial Items (Jun 2020) and the addendum, applies to this acquisition.
Addendum to FAR 52.212-1, Instructions to Offerors – Commercial Items (Jun 2020) Request for Quote (RFQ) shall contain the following information: RFQ NUMBER; TIME
SPECIFIED FOR RECEIPT OF OFFERS; NAME; ADDRESS;CAGE CODE; TELEPHONE
NUMBER OF OFFEROR; TERMS OF THE EXPRESSED WARRANTY; ANY DISCOUNT
TERMS AND ACKNOWLEDGEMENT OF ALL RFQ AMENDMENTS (if applicable).
Quotes must indicate quantity, unit price and total amount for each item. Offerors shall include a statement specifying the extent of agreement with all terms, conditions and provisions included in the RFQ. Quotes shall also contain all other documentation specified herein.
Addendum to FAR 52.212-1(c), Period for Acceptance of Offers. The paragraph is tailored as follows: “The Offeror agrees to hold the prices in its offer firm for 60 calendar days from the date specified for receipt of offers.”
8. The provision at FAR 52.212-2, Evaluation - Commercial Items (Oct 2014), and the addendum, applies to this acquisition.
Addendum to FAR 52.212-2, Evaluation – Commercial Items (Oct 2014) The Government will award a contract resulting from this RFQ to the responsible offeror whose offer conforming to the RFQ will be most advantageous to the Government, price and other factors considered.
A. To assure timely and equitable evaluation of quotations, offerors must follow the instructions contained herein. Offerors are required to meet all RFQ requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors. Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale.
The response shall consist of three (3) separate parts, Part I - Price Quotation and Part II – Technical.
B. Specific Instructions:
1. PART I - PRICE QUOTATION - Limited to one (1) page.
(a) Insert proposed unit and extended prices for each Contract Line Item Number (CLIN) listed above. The extended amount must equal the whole dollar unit price multiplied by the number of units.
(b) Offeror MUST have an Active Registration in the System for Award Management (SAM) located at www.sam.gov or www.beta.sam.gov.
2. PART II - TECHNICIAL QUOTATION - Limited to no more than five (5) pages.
The contractor must demonstrate that the proposed Core Laboratory solution meets all the minimum requirements in the RFQ Instructions and Performance Work Statement (PWS).
Technical Factors must demonstrate that the proposed contractor meets all minimum requirement listed in the PWS and those Factors listed below:
Factor I –General Equipment/System Requirements
• Instruments and assays are FDA cleared/approved (Vendor must provide proof of certification).
• No FDA warning letters issued over the past 24 months.
• Must have an automated microbiology platform capable of identification and susceptibility testing.
• Must be associated with a Matrix Assisted Desorption/Ionization Time of Flight (MALDI-
TOF) Mass Spectrometer.
• Must be capable of processing 50 panels per day. The contractor will provide enough instrumentation, reagent and consumables to complete approximately 6,000 panels per year.
• Must be able to complete ID panels in an average of 8 hours not to exceed 24 hours.
• Must be able to ID AND perform ASTs on minimal organism colonies from aerobic, throat, blood, stool, respiratory, and urine specimens/cultures
• Must have specialty identification panels for Yeast, Anaerobes, Haemophilus, and
Neisseria.
• Must have the capability of reloading sample, reagent and supplies without interruption from sample processing
• Include the most current and up to date CLSI breakpoints on all susceptibility tests.
• No on-board reagents
• Instrumentation must have a closed system to avoid leakage and contamination.
• All equipment must be on the Approved Products List (APL) and must be able to obtain an interim Authorization to Operate (ATO) within 30 days for networked or standalone information technology to receive, process, store, display, or transmit Defense Health Agency (DHA) information.
• Electrical Requirement: 120V
• Operating Temperature: 15°C ‐ 30°C
• Operating Humidity: Between 20°C‐90°C, relative, non‐condensing
• A printer with the capability of printing a patient report with patient demographic information that includes minimally the patient's name/accession number is required.
• An uninterruptible power supply (UPS) for each instrument provided under this contract http://www.sam.gov/ http://www.beta.sam.gov/ must be provided by the vendor and included in the total package quote as a line item.
• The total equipment package, per specifications, to be installed will not negatively impact the functionality/operations of that laboratory, nor will it require significant and/or costly infrastructure changes, as deemed by laboratory management
Factor II –Analyzer Platform
• The contractor must provide a test system at David Grant Medical Center for the 60TH
MDTS/SGQC DGMC Laboratory staff to use. The analyzer(s) must meet the performance characteristics for accuracy and precision as defined by the 1988 Clinical Laboratory Improvement Act (CLIA). Data quality for the US Food and Drug Administration (FDA) released library must have a correct Species/Complex ID rate of greater than or equal to 95% for routine organisms, as demonstrated by the FDA 510K Summary. Equipment must function with a 95% up-time and ability to perform patient testing.
Factor III – Reagent Minimum Requirements
• All reagent kits must be FDA approved and will exhibit at least a 90% agreement rate.
Upon request, sufficient reagents and samples shall be provided by the contractor for in‐house evaluation of each reagent. Following evaluation, each test kits shall exhibit at least 90% agreement with test kits currently in use in the Microbiology Laboratory. The contractor will be the sole manufacturer and distributor of reagents. Testing Panels must have a long shelf life (>9 months). The contractor will provide assistance troubleshooting any issues that result from College of American Pathologist (CAP) proficiency tests. The contractor shall comply with all FDA regulations, the current CAP checklist, and Clinical Laboratory Standards Institute (CLSI) recommendations.
Factor IV – Mass Spectrometer Instrumentation Minimum Requirements
• Must be associated with a Matrix Assisted Desorption/Ionization Time of Flight
(MALDI‐ TOF) Mass Spectrometer.
• Auto‐discrimination (reads more than one bar code symbology)
• Must have a robust FDA approved data base to include esoteric organisms without additional cost (numerous aerobic gram pos/neg, anaerobic, fungal/yeast, haemophilus, mycobacterium).
• Must have a database with a large number of Research Use Only organisms that are being consistently submitted to the FDA for diagnostic clearance. Once organisms are cleared they will be available in the FDA approved data base at no additional charge.
Factor V – Middleware Instrumentation Minimum Requirements
• Must have auto‐verification capabilities.
• Must have a bidirectional laboratory information system that is compatible with
Medical Health System Genesis (Cerner Millennium) or the current LIS system of the DGMC laboratory.
• Must provide a seamless interface link between the Biochemical Identification Instrumentation, Antibiotic Susceptibility Instrumentation, Blood Culture Instrumentation and MALDI‐TOF Mass Spectrometer Instrmentation.
• Operating systems will run on Windows 7 or newer (equivalent) and will be upgraded as necessary to ensure system is maintained in accordance with Air Force guidance. Will not install any system with Windows XP.
• Must provide middleware server for DGMC Laboratory.
Factor VI – Technical Features regarding Equipment/Services
• Contractor must assess equipment, work flow, workload volumes and space to meet the individual site requirements with minimum costs.
• Customizable reports (e.g. QC, Patient and Workload Reports, antibiograms)
• Audit trail capability (e.g. Operator ID, Panel Type, Date Placed on Instrument, etc.)
• All gram positive and gram negative identification results must not be held for greater than 24 hours from the time they are placed on the analyzer.
• All organism identification results must be objective without technician manipulation

Evaluation Factors for Award 52.212-2 Evaluation-Commercial Items. (Oct 2014)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
The following factors shall be used to evaluate offers:
(1) Price
(2) Technical Technical requirements are significantly more important than price when being evaluated.
(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

Addendum to FAR 52.212-2 EVALUATION - COMMERICAL ITEMS (COT 2014) BASIS FOR CONTRACT AWARD: This is a competitive best value requirement in which competing offerors' technical requirements will be evaluated on a basis significantly more important than price. By submission of its offer in accordance with the instructions provided in the addendum to clause FAR 52.212-1, Quotations Preparation Instructions, the offeror accedes to the terms of this model contract and all such offers shall be treated equally except for their prices.
The evaluation process shall proceed as follows:
A. Initially offers shall be ranked according to price, including options prices. An offeror's proposed prices will be determined by multiplying the quantities identified in the Bid/Pricing mailto:roberto.escobedo.1@us.af.mil
Schedule by the unit price for each item to confirm the extended amount of each. Copies of the Bid/Pricing Schedule will be necessary in order to submit an offer for each option year.
B. Technical Acceptability. Initially, the government will evaluate the technical requirements and how each factor was addressed. A determination will be rendered as to whether the technical requirement is evaluated as Acceptable or Unacceptable as follows:
Acceptable - Quotation meets the minimum requirements of the solicitation.
Unacceptable - Quotation does not meet the minimum requirements of the solicitation. Note: Technical requirements rated unacceptable will not be evaluated further.
9. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items (Jan 2017), Alternate I (Jun 2020), with their offer, or the offeror shall complete paragraph (b) of FAR 52.212-3, if the offeror has completed the annual representations and certifications electronically at http://www.sam.gov.

Funds are not presently available for this contract. The Government’s obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer.

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days.

52.217-9 Option to Extend the Term of the Contract. 2000-03 As prescribed in 17.208(g), insert a clause substantially the same as the following:
Option to Extend the Term of the Contract (Mar 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 66 months.

13. The Defense Priorities and Allocations System (DPAS) is not applicable to this acquisition.
14. Delivery date shall be 30 days after contract award.
15. All questions regarding this acquisition are to be directed ONLY to the 60th Contracting Squadron personnel. Questions will be accepted until 12 pm Pacific Standard Time, Monday 11 June 2021. After this date, no other questions will be allowed to be submitted nor will they be answered by the Government. Questions shall be in writing and sent by email to:
