RFQ ATTACHMENT 3 – ASSESSOR PAST PERFORMANCE QUESTIONNAIRE
Instructions:
· Sections I through III are to be completed by the Quoter/Major Subcontractor and provided to the Assessor (Government personnel who had experience with the Quoter/Major Subcontractor on a relevant contract referenced on RFQ Attachment 3). The Quoter DOES NOT submit this attachment to the soliciting agency, instead the Quoter sends this to the assessor after completing I – III, then the assessor is to complete the instructions below and then submits this directly to the soliciting agency.
· The Assessor, in turn, will verify Sections II and III, review Section IV, and complete Section V of this Questionnaire. The assessor will submit the completed Questionnaire directly to the Marine Corps Installations National Capital Region – Regional Contracting Office, at monica.curley@usmc.mil and desire.nana.civ@usmc.mil

| The Marine Corps is seeking genealogy support services to locate living descendants of unaccounted for Marines from past conflicts when the Defense POW/MIA Accounting Agency believes that they have recovered remains or have remains of unaccounted Marines in their Identification Laboratory, and professional genealogy support required to help the Marine Corps achieve this mission. The contractor shall be responsible for locating and providing contact information for the closest living family member. The objective is to locate and provide contact information of the closest living family member, referred to as the primary next of kin. Additionally, the requirement is to provide contact information on living DNA eligible family members for approximately 2700 Marines unaccounted for from World War II and approximately 149 Marines unaccounted for from the Vietnam War, the Korean War and the Cold War, for the purpose of collecting Mitochondrial DNA (mtDNA), paternal (Y-DNA), and autosomal/nuclear (auDNA - children of service member) for FRS comparisons, and to locate family members of Marines previously recovered and identified from the Vietnam War, the Korean War, the Cold War, and World War II when additional remains are recovered and identified. The critical performance requirement for this effort is to provide the labor, supplies, materials, and facilities to assist the Casualty Section in locating family members when additional remains are recovered and identified. |
