This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular: FAC 2023-04 (Published in the Federal Register June 2, 2023).

AHRQ’s Mission The Agency for Healthcare Research and Quality's (AHRQ) mission is to produce evidence to make health care safer, higher quality, more accessible, equitable, and affordable, and to work within the U.S. Department of Health and Human Services and with other partners to make sure that the evidence is understood and used. Patient Safety is AHRQ’s priority.

Teamwork and Communications Skills to Improve Patient Safety Communication gaps among health care providers are linked to negative patient outcomes and compromise patient safety. Further, teamwork is considered an essential component to safer healthcare. Developing and strengthening teamwork among health care personnel is a key initiative within the patient safety domain and can transform the culture within healthcare.

Patient safety experts report that communication and other teamwork skills are necessary for the provision of quality healthcare and for the prevention and mitigation of medical errors and patient injury and harm. Communication and other teamwork skills are major components of the TeamSTEPPS, an evidence-based patient safety program. Use and adoption of TeamSTEPPS, that trains on improving teamwork, can optimize patient outcomes and lessen burden on healthcare workers.

Evolution of the TeamSTEPPS Program In 2006 the Agency for Healthcare Research and Quality (AHRQ) and the Department of Defense developed Strategies & Tools to Enhance Performance and Patient Safety, or TeamSTEPPS®, aiming to produce highly effective medical teams by utilizing tailored concepts and resources to achieve the best clinical outcomes. TeamSTEPPS® was adopted from similar work that originated in the airline industry and US military. Research shows that TeamSTEPPS® has significantly contributed to improved communication and teamwork among

Strengthening coordination of care, hand-offs, improving communication and overall teamwork among health care providers in all settings continue to be a steadfast goal. In 2022, AHRQ released the TeamSTEPPS® for Diagnosis Improvement Course which applies the TeamSTEPPS® framework to the specific problem of diagnostic error.

In 2023, the TeamSTEPPS Program underwent significant update.

TeamSTEPPS Program Components TeamSTEPPS is grounded in team structure and four teachable-learnable skills:

Communication, Team Leadership, Situation Monitoring, and Mutual Support. The TeamSTEPPS® curriculum update created a consolidated curriculum that addresses these skills through four (4) modules. These interconnected skill sets contribute to the overall goal of highly reliable, safe, efficient, and patient-centered care teams. The updated curriculum provides unique guidance for various healthcare roles that are all part of the team including administrators, experienced trainers, frontline care providers, new trainers, professional trainers and patient and their caregivers. The TeamSTEPPS® curriculum is in an online format. As part of the update, three new Training Units and three video-based Patient Story about their encounters with the healthcare system have been added to the curriculum. The AHRQ TeamSTEPPS® website also includes evaluation tools to assess or quantify the TeamSTEPPS® impact as well as a research library and the TeamSTEPPS app.

Given the advancements in health information technology, changes in how care is delivered and a recent emphasis on engaging patients and families as members of the healthcare team, the TeamSTEPPS curriculum was significantly refreshed and made into a singular comprehensive program to better support teams in improving their communication skills and collaboration.

Current Needs For nearly two decades, TeamSTEPPS® has been utilized by the healthcare industry in various settings (hospitals, outpatient clinics, nursing homes, surgery centers, among others). However, since 2017, AHRQ has not supported formal TeamSTEPPS training delivered in a wide-ranging and organized mode across settings and audiences. Further, an assessment of skills, attitudes and behaviors is critical to inform how TeamSTEPPS augments healthcare teamwork in delivery of safer and of higher quality care. Such assessment of the TeamSTEPPS program has not been conducted to date.

Since the area of diagnostic safety is one of AHRQ top priorities, the only standalone TeamSTEPPS segment is the new TeamSTEPPS® for Diagnosis Improvement Course.

With the newly updated TeamSTEPPS curriculum including most of TeamSTEPPS components revised as well as new introduced, there is an opportunity to deliver TeamSTEPPS training across healthcare settings in a uniformed and systematic fashion. Afterwards, the assessment for change in individuals and teams is necessary to examine the degree to which the updated TeamSTEPPS program enhances users’ experience, improves the teams’ effectiveness, streamlines team communication and overall increases healthcare professionals’ commitment to interdisciplinary teamwork.

The TeamSTEPPS training is needed in different settings of various large and small healthcare and healthcare-related organization and institutions around the country.

Objectives The primary objective of this project is to implement the updated TeamSTEPPS training curriculum in at least 55 healthcare organizations and the new TeamSTEPPS® for Diagnosis Improvement Course in at least 60 healthcare organizations. This training should be provided at no cost to these organizations.

The secondary objective is to assess for knowledge, skills, attitudes and organizational change and document the findings. The TeamSTEPPS implementation in the form of training and technical assistance is to support the dissemination, adoption and use of TeamSTEPPS® by health systems and healthcare organizations, professional organizations, educational associations, educational institutions nationwide (e.g. medical and nursing schools), and Quality Improvement Organizations (QIOs). The assessment will test the overarching short and long-term impact of the TeamSTEPPS program.

The two TeamSTEPPS (TS) components for implementation and assessment include 1.

updated TeamSTEPPS curriculum to optimize patient safety and quality outcomes, and 2. the new TeamSTEPPS® for Diagnosis Improvement Course to decrease medical errors and patient harm. It is essential that the current and future healthcare workforce is trained as that may be key to uptake and adoption of TeamSTEPPS concepts and continuous improvement of teamwork in support of safe care delivery and amelioration of healthcare workforce burnout.

This project shall implement TeamSTEPPS training and conduct testing across various healthcare organizations and institutions in the United States including:

• at least 75 health care systems and affiliated primary care medical offices,

• at least 25 medical and nursing schools,

• at least 15 private sector institutions that offer TeamSTEPPS program.

The aim is to engage both connector groups (e.g., American Hospital Association, Quality Improvement Organizations, Quality Innovation Networks, practice facilitators for primary care offices, etc.) and different, small and large, healthcare institutions (e.g., rural and urban hospitals).

The items specified for delivery below are subject to the review and approval of the Contracting Officer Representative (COR) before final acceptance. The contractor shall make revisions as deemed necessary by the COR. Days mean calendar days.

Revised Section 508 Standards apply to Information & Communication Technology (ICT) procured, maintained, or used by the Federal Government. The standards were issued under Section 508 of the Rehabilitation Act which requires access to ICT by people with physical, sensory, or cognitive disabilities. This requirement extends to both federal employees and members of the public. Examples of ICT include (but are not limited to) computers, telecommunications equipment, software, websites, and electronic documents. Accessibility is the law and provides for inclusion. Instructions regarding 508 can be found in this contract in Section H; “Information & Communication Technology Accessibility” and “AHRQ Publishing and Communications Guidelines” at https://www.ahrq.gov/policy/electronic/about/policyix.html. Web sites, web tools, and/or web services (any functionality) that are new, redesigned, modified, or added cannot go-live until formal approval has been received from AHRQ’s Office of Communications. 4.1 Task Order Officer

The Task Order Officer is responsible for: monitoring the contractor’s technical progress, including the surveillance and assessment of performance and recommending to the contracting officer changes in requirements; interpreting the statement of work and any other technical performance requirements; performing technical evaluation as required; performing technical inspections and acceptances required by this contract; and, assisting in the resolution of technical problems encountered during performance.

4.2 Contracting Officer

A Contracting Officer is the only person with authority to act as an agent of the government under this contract. Only a Contracting Officer has authority to: direct or negotiate any changes in the statement of work; modify or extend the period of performance; change the delivery schedule;

authorize reimbursement to the contractor of any costs incurred during the performance of this contract; or, otherwise change any terms and conditions of the contract.

4.3 Key Personnel

Pursuant to the Key Personnel clause incorporated in Section 6 of this purchase order, the following individual(s) is/are considered to be essential to the work being performed hereunder:

The clause above contains a requirement for review and approval by the Contracting Officer of written requests for a change of Key Personnel reasonably in advance of diverting any of these individuals from this contract. Receipt of written request at least 30 days prior to a proposed change is considered reasonable.

4.4 Invoice Requirements

(1) The Contractor shall furnish the following MINIMUM information in support of costs invoiced:

a. Contractor Information – include organization name and address as it appears on the contract (or task order/BPA call as applicable). Include the TIN number

b. Contract Information – include the contract number and if applicable, the task order or BPA call number

c. Invoice Information – include the invoice number, which shall be a unique number specific to each invoice. Letters and numbers may be used. An invoice number may only be used one time regardless of the number of contracts or orders held by an organization. Duplicate invoice numbers are not allowed and will result in invoices being returned unpaid.

d. Task Number/Description/Milestone – (if applicable) identify the task number, milestone and/or description being billed

e. Current amount invoiced – by contract item/task and total dollar amount of this invoice

f. Cumulative amount invoiced – by contract item/task and total dollar amount

g. If FAR clause 52.219-14, “Limitations on Subcontracting” is included in the contract, provide calculations that demonstrate compliance with the limitation.

At a minimum, provide current and cumulative amounts for prime and subcontractors, and the percentage of the total for each.

4.5 Payment Schedule

The contractor shall provide the services/supplies as set forth in the Statement of Objectives and Deliverables, and will submit invoices in accordance with the following Milestone Payment Schedule:

4.6 Quality Assurance Surveillance Plan (QASP)

To be completed by the offeror and inserted into the purchase order at time of award.

5. SPECIAL REQUIREMENTS

5.1 Release and Use and Copyright of Data First Produced From Work Performed Under This Contract

(a) Release and Use – Data first produced in the performance of the Contract. As permitted in FAR 52.227-17, the provisions of this Section shall apply to any release or use of data first produced in the performance of the Contract and any analysis, tools, methodologies, or recorded product based on such data.

(b) Release and Use – Requirements related to confidentiality and quality. To ensure public trust in the confidentiality protections afforded participants in Agency for Healthcare Research and Quality (AHRQ)-supported research, AHRQ requires and monitors compliance by its contractors with section 934(c) of the Public Health Service Act (PHS Act) (42 U.S.C. 299c-3(c)), which states in part that

No information, if the establishment or person supplying the information or described in it is identif iable, obtained in the course of activities undertaken or supported under this title, may be used for any purpose other than the purpose for which it was supplied unless such establishment or person has consented...to its use for such other purpose. In addition to this requirement, section 933(b)(1) of the PHS Act (42 U.S.C. 299c-2(b)(1)) requires AHRQ to assure that statistics and analyses developed with Agency support are of high quality, comprehensive, timely, and adequately analyzed. Accordingly --

(1) prior to the release or use of data based upon work performed under this Contract, the Contractor agrees to consult with the Contracting Officer’s Representative and Contract Officers regarding the proposed release or use. The Contractor will in good faith consider, discuss, and respond to any comments or suggested modifications that are provided by AHRQ within two months of receiving the proposed release or use.

The purpose of such consultation is to assure that:

(A) identif iable information is being used exclusively for the purpose(s) for which it was supplied or appropriate consents have been obtained;

(B) the confidentiality promised to individuals and establishments supplying identif iable information or described in it is not violated; and

(C) the quality of statistical and analytical work meets the statutory standards cited above.

(2) The Contractor must satisfy conditions (1)(A) and (1)(B). At the conclusion of any consultation required by paragraph (b)(1) above, if AHRQ and the Contractor cannot agree that a proposed use or release satisfies condition (1)(C) above:

(A) the research professional at the Contractor responsible for the quality of the Contract work will, in advance of any release or use of such data, certify in a letter to the Contracting Officer what differences of opinion cannot be resolved regarding the statutory standards referenced in condition (1)(C) and the basis for Contractor assertions that these standards have been met; and

(B) the Contractor must print prominently on the release or other product, or on any portion that is released, or state prior to any oral presentation or release of such material, the following disclaimer:

(d) Copyright – Data first produced in the performance of the Contract. Subject to the terms of this Section regarding release and use of data, AHRQ, through its Contracting Officer, will grant permission under FAR 52.227-17(c)(1)(i) to the Contractor to establish claim to copyright subsisting in scientif ic and technical articles based on or containing data first produced in the performance of this contract that are submitted for publication in academic, technical or professional journals, symposia proceedings or similar works. When claim to copyright is made, the Contractor shall affix the applicable copyright notice of 17 U.S.C. 401 or 402 and acknowledgment of Government sponsorship (including contract number) to the data when such data are delivered to the Government, as well as when the data are published or deposited for registration as a published work in the U.S. Copyright Office. In such circumstances, the Contractor hereby agrees to grant to AHRQ, and others acting on its behalf, a paid-up, nonexclusive, irrevocable, worldwide license for all such data to reproduce, prepare derivative works, distribute copies to the public, and perform publicly and display publicly, by or on behalf of AHRQ. A description of this license will be incorporated into the copyright notices required above.

(e) Subcontracts. Whenever data, analyses, or other recorded products are to be developed by a subcontractor under this Contract, the Contractor must include the terms of this clause in the subcontract, without substantive alteration, with a provision that the subcontractor may not further assign to another party any of its obligations to the Contractor. No clause may be included to diminish the Government’s stated requirements or rights regarding release or use of products or materials based on data derived from work performed under this contract.

5.2 Lack of Compliance with Requirements for Release or Use

Failure to submit materials for statutorily mandated confidentiality and statistical and analytic quality reviews as required by Section 5.1 of this task/purchase order will be viewed as a material violation and breach of the terms of this contract, as the requirements of this provision are necessary for AHRQ to carry out its statutory obligations and responsibilities. Records of the Contractor’s performance, including the Contractor’s performance pertaining to this task/purchase order, will be maintained in AHRQ's Contracts Management Office and will be considered as an element of past performance which is part of all subsequent competitive task/purchase order quote reviews.

5.3 Subcontracts

The contractor shall include the requirements of all Section 5 clauses in all subcontracts. These requirements are to be included without substantive alteration, and no clause may be included to diminish these requirements.

5.5 AHRQ Publishing and Communications Guidelines

All publications and communications resulting from this contract shall comply with the AHRQ publications and communications guidelines. Products will be reviewed to ensure compliance with these guidelines. Deliverables that are non-compliant are not acceptable.

The AHRQ Publishing and Communications Guidelines can be found at http://www.ahrq.gov/research/publications/pubcomguide/. This will assist the contractor in this effort by coordinating the work with AHRQ Office of Communications (OC).

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplif ied acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records-Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after f inal payment under this contract or for any shorter period specified in FAR subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

(a) Definitions. As used in this clause— Payment request means a bill, voucher, invoice, or request for contract f inancing payment with associated supporting documentation. The payment request must comply with the requirements identif ied in FAR 32.905(b), “Content of Invoices” and the applicable Payment clause included in this contract.

(b) Except as provided in paragraph (c) of this clause, the Contractor shall submit payment requests electronically using the Department of Treasury Invoice Processing Platform (IPP) or successor system.
