QUESTIONS

RFP-P05051100003

Federal Medical Center (FMC) Carswell

I. Solicitation Format

Attachment 1 - Pages 11 and 15

1. There appears to be duplicate numbers for Output #4 on the above referenced pages. These do not match the output references on Page 3.

II. Technical Questions

Attachment 1 .. Output #2 .. Page 4 & 5 (Paragraph 1) & Page 7 (Paragraph 3)

1. Does the language under Output #2 enable the contractor to utilize physician extenders (e.g., PAts, Midwives, and/or Nurse Practitioners), who have appropriate educational qualifications, experience, licensure and credentials, to provide community standards of care for Community-Based Services and Institution-Based Services?

Attachment 1 - Output #2 - Community-Based Services .. Page 5

1. The BOP's requirement to provide access to specialty services in the community within 14 days from the date of referral is not consistent with community standards.

intends to meet the medical urgency requirements of inmate patient referrals; however, the general scheduling requirement for specialty care for commercial insurance companies (BC/BS, United Healthcare and Aetna) and other federal agencies (DOD TRICARE and VA) is 30 days from the initial consult.

requests the BOP to amend this target deadline to reflect 30-days from the initial consult. This language change will also be consistent with the recently awarded FCI Fort Worth contract.

Attachment 1- Output #3 - Dialysis Services - Pages 9 &10

1. The performance expectations for on-site dialysis services require the contractor to perform user maintenance labor on the Government's dialysis equipment. Does "user maintenance labor" apply and is it limited to typical treatment maintenance that would be required on dialysis equipment before and after conducting a dialysis treatment? Does this definition also include monthly preventative maintenance and repairs on the Government's dialysis equipment? Please clarify.

Attachment 1- Output #4 -Invoice Definitions - Pages 11 & 12

1. Please explain why the BOP has restricted contractors from submitting no more than 50 individual medical claims as an invoice for services rendered for Line Items 1a, 1b, 1c, 2a, 2b, and 2c.

Attachment 1 - Output #5 - Medical Records - Page 15

1. The solicitation requires the contractor to provide health care services which conform to community standards and other applicable regulations, including all specialty services and on-site after-duty hour emergency, emergent, and primary care services. It also requires the contractor to manage medical record information to promote the continuity of cafe provided to the inmate population at FMC Carswell.

It is understood that the Bureau has recently developed and implemented a Bureau wide electronic medical record system (BEMR) to document and track the health care services provided to its inmate population. .

Considering the community standard of care requires the availability of historic clinical data to help facilitate the provision of health care services, will the BOP allow contract providers access to BEMR while on-site at FMC Carswell? Will the BOP also provide electronic interface and/or remote access capability between BEMR and off-site contract physician offices and contract facilities?

Attachment 1 - Section IV - Infants Born to Inmates - Page 16

1. The solicitation requirements state the FMC "may" assume financial responsibility of the medical expenses for a child born to a female inmate. Please provide the criteria that will be used by the BOP to determine when they will assume financial responsibility of medical expenses for a child born to a female inmate.

The solicitation requirements further state the FMC's financial responsibility may be extended for a limited time for "extenuating circumstances on a case-by-case basis."

Please explain what types of infant cases the BOP will consider to be under the category of "extenuating circumstances."

2. Considering the fact the contractor has no official contact with the inmate family, guardian, or inmate, how will the BOP expect the contractor to coordinate the application for financial assistance (e.g., Medicaid) for the infant in the event the infant's medical needs exceed the BOP's authority for financial responsibility?

Please explain the BOP's role and responsibility, including the FMC staffing support, which will assist the contractor, and its agents, in obtaining the necessary signed documentation from the inmate family, guardian, and/or inmate to apply for such financial assistance. .

2. Since the Volunteers of America 0/0A) currently operates a corporate-owned contract facility in Fort Worth to support the BOP's Mothers and Infants Nurturing Together Program (MINT), will the BOP consider the VOA to be an authorized "optional user" of the contract awarded under this solicitation? If so, please specify any additional performance requirements and operational procedures that will need to be followed by the contractor to support this MINT program.

Answer: If no other resource can be compelled to pay, the BOP will assume financial responsibility as identified on Page 16 of the Performance Work Statement. If an infant requires an extended stay in the NICU/Hospital for any medical reason, the BOP Regional Director may approve an extra 7 days of medical coverage to be paid by the BOP (a total of 10 days for vaginal birth and 14 days for a Caesarean birth). If the infant still requires further medical stay after the 7 day extension, the contractor, subcontractor, and FMC will work together to apply for Medicaid coverage which shall be retroactive to birth.

Regardless of the state in which the infant will reside, the infant is considered a Texas resident because of the length of stay is undeterminable.
