HEALTH FEDERALLY FUNDED RESEARCH & DEVELOPMENT CENTER (FFRDC)

FOR

CENTERS FOR MEDICARE & MEDICAID SERVICES (CMS)

STATEMENT OF WORK

I. PURPOSE AND BACKGROUND

Purpose

The Centers for Medicare & Medicaid Services (“CMS” or “Agency”) is seeking a qualified Federally Funded Research and Development Center (“FFRDC” or “Contractor”) to sponsor in order to provide independent strategic guidance and neutral evaluation of efforts to better align competing priorities with limited resources, and to facilitate the modernization of key business processes and support of systems and their operations. Specifically, CMS is seeking an FFRDC with technical expertise and experience in providing strategic support activities that align CMS business, policy, technology and operations needs. The emphasis of these activities focuses on the enterprise view and good customer service as manifested in Agency strategic and tactical planning; conceptual planning and proofs-of-concept; assessing business requirements and technical business architecture needs, assessing new technologies; and providing medical and technical innovation, and continuous improvement strategies.

The FFRDC’s successful performance of its tasks requires that a unique special relationship exist between the FFRDC and CMS. The FFRDC will provide services to address specific long-term business and technical needs that cannot be met by any other contractor. That relationship includes:

• Comprehensive knowledge of CMS needs – mission, culture, expertise and institutional memory regarding issues of enduring concern to CMS.

• Adaptability – ability to respond to emerging needs of CMS and anticipate future critical issues.

• Objectivity – ability to produce thorough, independent analyses to address complex technical and analytical problems.

• Freedom from conflicts of interest and dedication to the public interest – independence from commercial, shareholder, political, or other associations.

• Long-term continuity – uninterrupted, consistent support based on a continuing relationship.

• Broad access to sensitive government and commercial proprietary information – absence of institutional interests that could lead to misuse of information or cause contractor reluctance to provide such information.

• Quick response capability – ability to offer short-term assistance to help CMS meet urgent and high-priority requirements.

• Comprehensive knowledge of the health care policy, business, delivery, quality improvement, and IT landscape with an emphasis on relationship management across entities and the demonstrated ability to bring the Agency together to resolve enterprise challenges.

Background

CMS is an operating division of the US Department of Health and Human Services (HHS), responsible for administering Medicare, Medicaid and the Children’s Health Insurance Programs. These programs provide health insurance coverage for over 90 million Americans, making CMS the largest health care payer in the country, managing expenditures of over $800 billion per year. The recent passage of new legislation providing for Health Information Technology for Economic and Clinical Health (HITECH) in the American Reinvestment and Recovery Act (ARRA), and health care reform included in the Affordable Care Act (ACA) will impact CMS in dramatic ways.

CMS will lead the transition to paying for quality care that is measurable in terms of improved outcomes and reduced costs, and away from paying for health care services based on volume. With timely, complete, and accurate data, CMS will address today’s health care challenges and have the ability to support future healthcare service delivery innovations. In order to continue to build a transformative and innovative healthcare delivery system, CMS must focus its improvement efforts on optimizing its core capabilities at the enterprise level within the context of the related external environmental factors (e.g., regulatory, political, stakeholders financial, technical, etc.). By focusing on an enterprise approach to optimization of various programs, initiatives, and operations, CMS will be in a much stronger position to design more-effective programs; implement streamlined business processes, and acquire strategic and cost-effective technology solutions for performing core functions.

Specifically, CMS wants to develop and implement capabilities to deliver:

• Payment flexibilities – CMS must transition to new payment methodologies integrating encounter, clinical, payment and outcome data while maintaining its massive Fee-For- Service (FFS) operation.

• Transparency into cost and quality drivers – CMS must offer modern analytical capabilities for cost and quality, supported by rich enterprise level databases.

• Provider Satisfaction – CMS program, billing, and eligibility information must be streamlined, making our systems and processes more informative and less burdensome for providers.

• Empowered beneficiaries – to improve health outcomes, CMS must promote a patient-centered focus on prevention and wellness, chronic care management and individual health responsibility. All three depend on the patient having access to information about coverage, access, and quality of care, allowing them to more fully engage in their health care.

• Timely, comprehensive program oversight – to improve CMS programs, there must be better oversight and data, encompassing a wide range of management activity that strive to control healthcare costs and improve quality for the Medicaid, Medicare, and Children’s Health programs.

II. SCOPE AND TASKING

Scope of the Contract

The Contractor’s overarching mission in support of CMS is twofold:

1. Leverage its special relationship with CMS to gain an expert level understanding of CMS current operating, regulatory, and information technology environment and financial constraints, as well as anticipating future changes, and identify opportunities for improvement in the delivery of CMS programs and operations by conducting focused studies and evaluations to guide conceptual planning and prototyping, research, program development, and information technology acquisition.

2. Provide CMS strategic, technical, and program management advice, guidance, and support services to assist the CMS with implementing various strategic initiatives resulting from recent legislation like ARRA and ACA, and Administration priorities such as the Open Government Directive (OMB M-10-06) and operational support as authorized per Section H.30.8 and as provided for in Federal Acquisition Regulation (FAR) Section 35.017-3.
