About this file
This is a Certified Disadvantaged Business Outreach Plan attachment for RFP 6220 issued by the State of Oregon, Oregon Health Authority (OHA), for technical bed need analysis of Certificate of Need applications. The attachment requires proposers to complete a comprehensive form documenting their engagement with certified disadvantaged business enterprises (DBEs), minority-owned businesses (MBEs), women-owned businesses (WBEs), service-disabled veteran-owned (SDV) businesses, and emerging small businesses (ESBs) as defined under Oregon Revised Statutes 200.055 and certified by the Oregon Certification Office for Business Inclusion and Diversity (COBID). Proposers must identify whether they hold any COBID certifications, list certified firms with which they have contracted in the past two years, identify non-certified firms that may qualify as minority-owned or disadvantaged entities, and disclose anticipated subcontracting opportunities.
For proposers identifying subcontracting opportunities, the attachment requires detailed descriptions of solicitation steps for certified firms, itemization of subcontracting opportunities with approximate dollar values, and willingness to report subcontractor identities and values to COBID upon contract award. Certified firm participation is mandatory with equal opportunity provisions, and submission of this outreach plan does not factor into scored proposal evaluations. The outreach plan must be submitted with the full proposal if subcontracting is anticipated.
