RFP-17-PHX-01 Preventive Maintenance and Repair Bio-Medical Equipment, PAO

Q&A

1. Q: Was there a previous contract for the service and if so, what was the contract number?

A: HHSI247201100278G

2. Q: Are all equipment currently operational?

A: All preventive maintenance inventories represent active medical equipment.

3. Can we submit just a spreadsheet that shows our pricing on each facility and total amount for 5 years? Not all your spread sheets?

A: For evaluations purposes, I want all offers to look the same.

4. I was hoping you would be able to explain to me what needs to be included in the inventory line items for pricing?

A: Basically state what you charge to PM the device for one year.

1) The easy ones will include one safety test per year.

2) If there needs to be multiple tests per year, state the total cost and add a comment column describing how many tests were included.

3) I will have specialty contracts for Imaging Equipment, and Fume Hoods, however, I still will require a

Chassis to Ground test. I do not expect you to perform PM’s.

4) Computerized Lab and Pharmacy equipment is still considered medical equipment. Therefore, include a

Chassis to Ground tests. Unplugging or turning off computers will not be required.

5) If there is device that you certified to test, state “No Bid” in the comments column and just perform the

Chassis to Ground test.

5. Are we to include a full service contract price covering all pm’s, all repairs and all parts or are we to only price PM labor and everything else will be billable as needed? The SOW is unclear and confusing.

A: The repair inventory is only a list of equipment that you may be called on to repair. This gives me the latitude to have you repair anything on this inventory even if you have to sub contract the work. It’s a funding source of money.

An individual task order will be generated for each repair stating the device, location and description of the failure. For an agreed check out fee, you will be asked to troubleshoot and provide a cost estimate to include parts, labor, and shipping to repair the device. If I accept your cost estimate, I will generate a modification to the task order to include the repair cost estimate. If I decline the cost estimate, you will be paid the agreed check out fee.

As stated in the previous PM paragraph, I need you to state the total cost to perform manufacturer recommended preventive maintenance the device for a year. Multi tests, No Bid, and Chassis to Ground testing only are credible options.

I have 12 institutions to cover in three states. Only five institutions have Biomed programs, seven institutions rely on myself for coverage. The five institutions with Biomed programs after reviewing your pricing will select the amount equipment they will test and give me the rest. The seven programs with no Biomed programs will participate 100%.

This allows me the flexibility to add or subtract participation percentages annually depending on staffing patterns
