3.0 SCOPE OF WORK
The South Dakota Department of Social Services (DSS), Division of Behavioral Health is seeking a qualified vendor to provide cost analysis and prospective payment systems (PPS) evaluation services as part of a Certified Community Behavioral Health Center (CCBHC) planning grant.
DSS is looking to initiate the project on November 1, 2025, and complete the primary deliverables of the project by December 31, 2025, which aligns with the end of the State’s currently awarded Planning Grant. Should an extension be granted from the federal funding authority, vendors can anticipate an extension to the timeline associated with this Scope of Work if needed.
Upon contract initiation, DSS will provide the selected vendor with a) a list of providers who have indicated interest in contributing to PPS rate evaluation efforts; b) preliminary cost reporting information and completed cost report templates from agencies that are considering provisional certification through the state; and c) a finalized scope of services to inform rate setting evaluation steps.
Applicants should demonstrate understanding, experience and expertise regarding PPS rate setting methodologies and options available based on Section 223 CCBHC Demonstration PPS Guidance, including experience working with state Medicaid systems and providers to develop the payment methodology. It is expected that qualified vendors will have experience in analyzing cost and utilization data, developing rate calculations, and developing reimbursement policies and procedures, as well as a demonstrated ability to engage and communicate with potential CCBHC providers about the PPS rate options. The selected vendor must ensure compliance with federal Centers for Medicare & Medicaid Services (CMS) guidelines and applicable South Dakota Department of Social Services (DSS) and South Dakota Medicaid policies.
DSS has convened a CCBHC steering committee of agency leaders actively working to formalize the Scope of Services and preliminary cost reporting information. In addition, DSS will convene a group of stakeholders inclusive of individuals with lived experience to review and provide feedback on selected services and approach. The selected vendor will participate in stakeholder engagement with both of these groups as requested by DSS and may be asked to deliver presentations regarding the data gathering, evaluation, analysis and recommendations.
Implementation of evaluation efforts and cost analysis will be conducted by the consultant in conjunction with the state team. The State expects the selected Vendor to develop and submit a Final Report for no later than December 31, 2025, inclusive of the following information:
• Executive summary.
• Background and purpose.
• Methodology and findings including a summary matrix of research of other states’ methodologies.
• Rate model recommendations of the “best” PPS model option which aligns most closely with South
Dakota’s objectives, scope of service, and regulations. Vendor must ensure full inclusion of direct, indirect, administrative, and overhead costs per federal and state standards.
• Fiscal impact analysis.
• Recommendations for provider education about recommended “best” PPS model.
• Recommendations for specific system changes needed to support the proposed PPS model, and criteria by which the state can evaluate rate rebasing for [period of time].
• Stakeholder engagement process summary.
• Summary matrix of statutory analysis.
3.1 Project Approach
3.1.1 Applicants must provide a narrative describing their overall approach to complete the Scope of
Work outlined in Section 3.0 in the timeline available for the work.
3.1.2 Applicants are expected to demonstrate in their response two potential paths for sustainability for future rate setting reviews. This should include a strategy that can be turned over to the state by the end of the two-month period to be used in the future for the state to review selected rates one year after implementation (see 3.4.2 & 7.2). Applicants should also demonstrate a plan for vendor-led review of rate setting activities for the one-year period after the initial rate setting plan is complete. Selected proposals will be approved provisionally for the vendor-led option subject to continued funding and satisfactory completion of the initial scope of work (see 7.3).
3.1.3 Vendors must provide a narrative describing their familiarity with providing the requested Scope of Work in similar settings to South Dakota, that which work with agencies providing services across large geographical regions in predominantly rural settings.
3.1.4 Offerors provide two proposed timelines: one outlining work through December 31, 2025, and one extending through May 31, 2026. Each timeline should specify deliverables and any additional activities that would be undertaken during the extended period. This information will assist the State in determining the feasibility of utilizing additional funding to continue supporting the work should a no cost extension not be awarded.
3.1.5 Vendors must plan to attend a project inception meeting within seven days of contract initiation
(virtual or in person) and articulate to the State in its response what information would be needed to best guide the Vendor’s approach given the timeline and scope outlined in this proposal.
3.2 Conduct Data Gathering Activities
Vendor will create and implement data gathering methods to obtain pertinent information from public and private providers of care that would otherwise potentially be provided by a CCBHC in PPS rate determination efforts. All gathered data will remain the property of DSS after the completion of this project and will be used for the sole intent of this project unless otherwise approved in writing by DSS.
Proposals must provide responses or acknowledgement of the following requirements:
3.2.1 Proposed plan of data gathering methods.
3.2.2 Proposed implementation of data gathering methods and timeline to accomplish.
3.3 Evaluation of the Scope of Services against Prospective Payment Methods
Vendor will evaluate the provided scope of services recommended by the State and describe evaluation strategies that will be used to determine each of the following requirements:
3.3.1 Using state or agency-provided cost reporting data, model total costs associated with providing the required CCBHC service lines across each of the established PPS methodologies.
3.3.2 Time associated with providing the services identified within the state’s scope of services.
3.3.3 Analysis of anticipated cost structures as they would relate to State and Medicaid funding.
3.4 Recommendation for Rate Setting
Vendor will provide recommendations for rate setting options that best meet the needs of South Dakota and provide recommendations for annual updates of PPS rates as well as recommendations for methods to update base rates per Section 223 guidance. Proposals must provide responses or acknowledgements of the following:
3.4.1 Identification of the recommended PPS rate methodology and individual rates for scope of service elements that best meets the needs of South Dakota and rationale for that recommendation.
3.4.2 Planned approach to assessing the impact of PPS rate model changes on services and providers.
3.4.3 Provide action plan for evaluating PPS rates for consideration as the State continues into
Demonstration-period or similar efforts post-Planning grant.
3.4.4 Planned approach to recommending methods to update base rates per Section 223 guidance.
3.5 Project Management
RFP proposals must explain the vendor’s project management techniques and processes to include, at minimum, the following requirements:
3.5.1 A Preliminary Project Work Plan that describes the technical approach and steps to be taken in partnership with the State and agencies considering provisional certification for the duration of the project period.
3.5.2 A project management plan inclusive of regular, periodic project status reports and recommended intervals for those discussions.
3.5.3 RFP proposals must specify how the vendor intends to share data, reports, and other information such as utilization of a vendor provided portal, dashboard, or SharePoint or equivalent. The State typically prefers to utilize its licensed SharePoint or Teams site but will discuss as needed with selected vendor.
3.6 Additional Proposal Requirements
3.6.1 Proposal must include vendor’s availability to travel to South Dakota and timing by which the
Vendor proposes doing so. It is expected that project work activities can occur completely remotely; however, travel to South Dakota may be deemed necessary and required as determined by the State.
3.6.2 Proposals must describe their ability and proven history in handling special project constraints.
3.6.3 Proposals must describe the offeror’s resources available to perform the work, including any specialized services, within the specified time limits for this project.
3.6.4 Proposals must include names, resumes, and specialized expertise of key project staff.
3.6.5 Proposal must include a record of the vendor’s past performance, including price and cost data from previous projects, quality of work, ability to meet schedules, cost control and contract administration.
4.0 PROPOSAL REQUIREMENTS AND COMPANY QUALIFICATIONS
