About this file

This is an RFP Submission Checklist document for the City of Philadelphia, designed to assist applicants in preparing complete and compliant proposals for Professional Services RFP opportunities meeting or exceeding the formal RFP threshold. The checklist addresses compliance with submission deadlines, alignment with the Scope of Work requirements outlined in Section II of the RFP, inclusion of a Cost Proposal consistent with specified requirements, and adherence to format and content standards described in RFP Section III. Applicants must review all RFP and Contract Attachments, including Appendix A - General Provisions, and formally state any objections within their proposals, understanding that objections are not favored and may negatively impact evaluation. The related contract opportunity involves the City seeking proposals from qualified auditing firms specializing in Pennsylvania workers' compensation claims audits to audit the City's indemnity payments issued by third parties.

Applicants must submit Appendix C: City of Philadelphia Tax and Regulatory Status and Clearance Statement and a Statement of Financial Capacity, supplemented by audited or unaudited financial statements, disclosure of any bankruptcy filings within the past five years, or the most recent IRS Form 990 for non-profit organizations. All information required under Chapter 17-1400 of the Philadelphia Code, including local political contributions, must be disclosed through the applicant's eContractPhilly account. Optional submissions include Appendix D: Local Business Entity or Local Impact Certification and Appendix E: LGBTQ Applicant Opportunity Data. Submissions must be made electronically through eContractPhilly under the correct opportunity number, signed by an authorized signatory through the "sign and submit" function, with applicant name and Tax Identification Number matching across the eContractPhilly profile, application, and W-9 form. The checklist was revised in July 2025.
