About this file
This is an RFP Submission Checklist issued by the City of Philadelphia for Professional Services contract opportunities at or above the formal RFP threshold. The checklist serves as a compliance tool to ensure applicants submit complete, accurate, and timely proposals. Key submission requirements include adherence to the stated deadline, demonstration of how the applicant meets the Scope of Work requirements outlined in Section II, submission of a Cost Proposal consistent with RFP specifications, and compliance with overall format and content requirements detailed in Section III. Applicants must review the entire RFP and Contract Attachments, including Appendix A General Provisions, and must raise any objections to these provisions within their proposal submission, as objections raised later will not be considered.
The checklist requires submission of Appendix C (City of Philadelphia Tax and Regulatory Status and Clearance Statement), a Statement of Financial Capacity, and supporting financial documentation such as audited or unaudited financial statements, bankruptcy disclosures from the past five years, or IRS Form 990 for non-profit organizations. Applicants must disclose all information required under Philadelphia Code Chapter 17-1400, including local political contributions, through their eContract Philly account. Optional submissions include Appendix D (Local Business Entity or Local Impact Certification) and Appendix E (LGBTQ Applicant Opportunity Data). All proposals must be submitted electronically through eContract Philly under the correct opportunity number, signed by an authorized signatory through the submission platform, and must verify that the registered applicant name and Tax Identification Number match across the applicant profile, Appendix C, and W-9 form.
