About this file
This document is a Reference Check Form (Attachment D) for an Oregon state procurement opportunity through OregonBuys, specifically for Fisher and Grey Fox GPS Collars, issued by DAS Procurement Services on August 16, 2021. The form is part of a request for proposals (RFP) numbered S-63500-00013990 and requires offerors to submit completed reference check forms from two references as part of their proposal submission.
The reference check form is designed to evaluate potential vendors by having their previous clients rate the offeror's performance across multiple dimensions, including overall service quality, timeline adherence, customer responsiveness, and specific technical performance metrics related to GPS collar functionality. References will be asked to rate various aspects on a 0-10 scale, with questions focusing on service quality, timeline management, and technical performance such as battery life, transmission reliability, and accuracy of mortality identification. The agency will make three attempts to contact each reference, and failure to reach a reference will result in a zero score for that reference.
