This is a Price Schedule attachment (Attachment B) for RFP PR9120 issued by the City of Orlando Utilities Commission (OUC) in Florida for Anhydrous Ammonia Supply and Pump Out Services. The RFP solicits qualified suppliers to provide anhydrous ammonia delivery and unloading into designated OUC storage tanks at Stanton Unit 2 and Stanton Unit B, along with periodic onsite pump out services. Bidders must submit unit prices per short ton for both ammonia grades and per pound pricing where applicable, along with detailed pricing for pump out services including pump out fees, reclaim fees for 250-pound/59-gallon quantities, and portal-to-portal costs. The proposal must include the proposed frequency and index for any price adjustments, the calculation formula to be applied, and disclosure of any additional charges such as transportation or fuel surcharges. Fuel surcharges, if proposed, must be based on the Florida Department of Transportation Fuel and Bituminous Price Index and must include a detailed surcharge table showing base fuel prices and incremental surcharge amounts. Bidders may also propose payment discount options including 1% discount for payment within 20 days (net 30), 2% discount within 15 days (net 30), 3% discount within 10 days (net 30), or other alternatives. Hourly rates must be specified for regular weekday work, Saturday work, and Sunday/holiday work, plus any per-gallon charges for consumption exceeding the specified 250-pound/50-gallon limit.
