About this file
This file consists of standardized attachment forms for an intermediate procurement issued by the Oregon Department of Fish & Wildlife (ODFW) through OregonBuys (RFP #S-63500-00015493) for statistical consulting services related to mule deer winter range models in Eastern Oregon. The project requires a contractor to provide statistical analysis, written documentation of methods, and results regarding habitat use of mule deer at the Starkey Experimental Forest and Range. The solicitation includes mandatory forms for proposer disclosure exemptions, proposer information and certification, reference checks, disadvantaged business outreach plans, responsibility inquiries, and scored evaluation components. Proposals must be firm offers for 90 days following opening, and evaluation will be based on general qualifications (30 points), understanding of services/project (10 points), statistical director qualifications (30 points), cost sheet (15 points), and references (15 points) for a total of 100 points.
The document establishes certification and compliance requirements applicable to all proposers, including tax compliance, non-discrimination in employment and subcontracting practices, compliance with wage equity laws, exclusion list verification, and conflict-of-interest certifications. Proposers must demonstrate financial responsibility, relevant experience with similar contracts completed within budget and schedule, and current licenses and insurance to do business in Oregon. The procurement emphasizes outreach to certified disadvantaged business enterprises, minority-owned, woman-owned, service-disabled veteran-owned, and emerging small businesses for any subcontracting opportunities. A Pay Equity Certificate is required if the proposer employs 50 or more full-time workers and the prospective contract price exceeds $500,000. All certifications and representations made in proposals are subject to the Oregon False Claims Act, with false claims potentially resulting in liabilities and penalties.
