Title: Support for Polio Eradication and Vaccine Preventable Disease
Activities in Sub-Saharan Africa-AFRO Team
C.1.Background and Need
Polio eradication is a high priority, for the Center for Global Health (CGH), and for the entire
Agency. CDC’s Emergency Operations Center (EOC) was activated for the polio response in
November 2011.
The Global Immunization Division receives appropriated and other funds to support its vision of a world without vaccine-preventable disease, disability and death. This vision is also a CDC priority and a Winnable Battle. The division is currently expanding its activities in support of polio eradication and elimination, strengthening routine immunization systems, including improving the quality of data and the implementation of vaccine preventable disease surveillance in developing countries.
While CGH works to end polio in the two remaining endemic countries, the CDC Polio
Response is also working to stop outbreaks of circulating vaccine-derived polio viruses
(cVDPVs) in countries across the world where there are geographic pockets of unvaccinated children. The outbreaks of cVDPVs are impacting many countries in sub-Saharan Africa and represent a growing risk to polio eradication efforts. There is an urgent need to stop them rapidly and effectively.
To address this critical issue, the CDC Polio Response is continuing our contracting engagement in active outbreak responses and improving case detection in numerous at-risk countries with expertise in epidemiology and surveillance, emergency management, and outbreak response.
CDC-CGH-PEB is using various cooperative agreements (CoAgs) with the following partners/grantees to achieve the goals listed above.
GH001873 Task Force for Global Health (TEPHINET)
GH002172 African Field Epidemiology Network (AFENET)
GH001876 African Field Epidemiology Network (AFENET)
GH002001 UNICEF
GH002072 International Federal of the Red Cross and Red Crescent (IFRC)
The mission of the Polio Eradication Branch (PEB), Global Immunization Division (GID) in the
Center for Global Health, is to plan and implement immunization activities globally as an active emergency response, with the immediate focus on polio eradication efforts in Africa as well as on-site technical support at CDC headquarters in Atlanta, Georgia. The Division is mandated to support polio eradication activities and vaccine preventable diseases (VPDs) such as measles, in support of the goal set by the World Health Organization and the Director of the Centers for
Disease Control and Prevention of worldwide eradication of the polio virus to include:
1. Outbreak response, environmental surveillance, poliovirus containment, post-eradication planning, and polio program legacy activities.
2. Measles elimination and control.
3. Immunization systems strengthening, including routine immunization (RI) systems.
4. New vaccine introduction.
5. Assist MOH or other partner with workforce and information systems -related questions.
6. Improving the quality of immunization data collection/
7. Implementation of vaccine preventable disease (VPD) surveillance in developing countries.
The Global Immunization Division engages in these activities as part of the international effort to eradicate Polio in coordination with Ministries of Health (MoH), the Global Polio Eradication
Initiative (GPEI)) and UNICEF and/or projects implemented jointly with these organizations as part of the emergency activation for Polio via the CDC Emergency Operations Center (EOC).
While the geographic scope for these activities is currently focused on Africa, requirements change rapidly with changes in the epidemiology of the VPDs, particularly concerning polio and
VPD outbreak response. Resources must shift to address immediate, often urgent, field and research needs in all parts of the globe. This is addressed in section C.4.2. below under Surge
Capabilities.
The Strategic Information and Workforce Development Branch (SIWD) and the Polio
Eradication Branch (PEB) in the Global Immunization Division (GID seeks data management services to support several areas of work related to polio eradication and routine immunization.
For example, helping to implement serologic surveys to inform polio eradication efforts and improve routine immunization health facility surveys to evaluate new vaccine introduction and inform the polio endgame strategies, and assisting to complete Hepatitis B serosurveys.
The purpose of this requirement is to fulfill the Global Immunization Division’s (GID) roles and responsibilities within the Global Polio Eradication Initiative of providing technical support to priority countries with endemic or reestablished transmission of wild polio virus to intensify their efforts to eradicate polio. As the nation’s disease control and prevention agency, CDC must provide leadership to the international public health community to prepare for and respond to acts of health threats.
C.2. Project Objective
This contract will further GID’s mission and activities in informing the Ministries of Health on polio eradication strategies, VPD surveillance, polio campaign improvements, routine immunization implementation, best practices for data management, poliovirus containment and legacy activities. In addition, this contract supports the GPEI partners with CDC by providing a means for ‘substantial involvement’ as required by federal law.
C.3. Scope of Work
Services required include:
1. Assist Ministries of Health (MOH) and other international organizations (Non
Government Organizations (NGOs), and other Public Health organizations) with epidemiologic data analysis and to identify solutions to problems that arise in polio eradication and VPD program implementation and outbreak response.
2. Assist MOHs and other partners with planning and implementing activities to improve data management practices for vaccine preventable disease surveillance.
3. Assist MOHs and other partners in improving surveillance for diseases prevented by existing and new vaccines.
4. Supporting immunization activities, both routine and supplemental activities as well as acute flaccid paralysis (AFP) and polio case investigations.
5. Assisting in the development of technical and financial reports.
6. Collaborating with the Polio Eradication program, the Immunization, Vaccines and
Biologicals (IVB) program, and the Expanded Program on Immunization (EPI).
7. Providing technical advice to the Global Polio Eradication Initiative (GPEI).
8. Coordinating CDC responses and inputs and advising CDC regarding technical contributions to the Eradication and Outbreak Management Group (EOMG) at the United
Nations in New York, NY.
C.4. Tasks (Technical Requirements)
The Contractor shall provide services to CDC Headquarters, CDC Field Offices, and GPEI partners in various international and domestic locations. Historically, outbreak response has included activities in countries including, but not limited to, Angola, Cameroon, Central African
Republic, Chad, Democratic Republic of the Congo, Ethiopia, Guinea, Madagascar, Mali, Malawi, Niger, Nigeria, South Sudan, Sudan, and Uganda. For non-Surge, as outbreaks occur and as countries become certified Polio-free and transition away from polio eradication, services in additional countries and other VPD’s may be required.
The Contractor shall provide personnel, facilities, supplies (including personal protective equipment), equipment and services for performance of the following tasks:
1. The contractor will be required to complete a REDCap, (or other portable software), weekly activity survey, and a monthly activity summary from each Field Support Officer
(FSO), Epidemiologist, and Data Manager that captures information as agreed upon between the contractor and CDC. The monthly report summarize accomplishments, challenges encountered and solutions proposed, as well as any additional recommendations by country (including travel and Surge activities). Mid-year and Year-end Legacy Activity
Progress Reports will also be required (including travel and Surge activities ).
a) Provide evidence-based technical advice for implementation of polio eradication activities (such as immunization campaigns or supplemental immunizations) that are appropriate to the circumstances in endemic countries and/or countries with outbreaks of polio or other VPDs such as measles.
b) Provide advice in the strengthening of the AFP and VPD surveillance systems via coordination, monitoring and evaluation of surveillance activities. Includes travel to remote locations to conduct surveillance activities, which include: finding and reporting children with AFP, transporting stool samples for analysis (for the purpose of isolating and identifying poliovirus in the laboratory to map the genetic sequencing to determine the origin of the virus strain).
c) Conduct three- and six- month assessments of outbreak status, effectiveness of response and likelihood of continuing transmission of polio and other VPDs (e.g. measles).
Example: Use assessment data on unvaccinated children to develop outbreak response and polio campaign plans.
d) Conduct training for GPEI partners on the topics of evaluation and documentation of outbreak response activities to ensure that all lessons learned are effectively incorporated into current best practices.
e) Participate in activities designed to promote polio, measles and other emerging vaccine preventable disease eradication, routine immunization, data quality and surveillance of new vaccines. Attend meetings and webinars with international and local partners in order to stay abreast of the most current developments in the outbreak and status of worldwide eradication. Incorporate and share lessons learned and best practices developed during work conducted on CDC polio eradication projects with country and field staff. Example: Conduct Expanded Program on Immunization (EPI) reviews, surveillance assessments, data quality assessments, GIS and satellite mapping, microplanning support or other aspects of program improvement and implementation.
f) Analyze World Health Organization (WHO), Africa Regional Office (AFRO), or host country partner epidemiologic data as it relates to VPD surveillance, disease transmission and outbreaks for use by CDC in documentation, data management and legacy planning.
Example: Use epidemiologic data and polio expertise to identify high risk districts and help build capacity to vaccinate chronically unreached and under-vaccinated children.
g) Analyze epidemiological data and working with Ministries of Health (MOH) and other international organizations (NGOs, and other Public Health organizations) to create solutions to problems that arise in polio eradication and VPD program implementation and outbreak response. Example: Identify specific improvements and activities from
SIA’s that will be implemented to strengthen immunization practices at the district and national level.
h) Provide technical advice to CDC’s Strategic Information and Workforce Development
(SIWD) Branch of the Global Immunization Division (GID) regarding the training and field work of National Stop Transmission of Polio (NSTOP), and Stop Transmission of
Polio (STOP) teams. Example: Develop training materials and courses to train polio staff on enhancing the overall immunization surveillance system. Conduct refresher training for national immunization staff.
i) Provide technical advice to the Polio Eradication Branch management regarding data collection activities in operational research, including integration projects in polio endemic countries and countries with transmission of polio, in order to assist with the analysis of the data collection and surveillance efforts during SIAs and routine immunization activities in country.
j) Participate with Polio Eradication Branch team leads in development and implementation of field project plans addressing routine immunization, data quality or surveillance, outbreak investigation, and vaccination campaigns. Use historical polio data to improve outreach activities. Example: Use data on hesitant households/communities to develop and improve microplanning activities. Develop strategies to integrate polio surveillance with VPD surveillance. Engage with community service organizations to improve outreach activities.
2. Surge Labor:
Due to the nature of GID’s outbreak response work, there may be times that “surge support” would be needed to assist with division response activities related to emergency conditions or situations. The contractor shall be capable of responding to unanticipated demands for increased capacity of services within existing capabilities covered under this PWS that may require a “surge” in service efforts and/or resources.
The surge services are to be provided above and beyond what is required for on-going activities, specifically related to an emergency Vaccine Preventable disease outbreak or other emergency response that requires VPD expertise (for example, natural disasters such as:
pandemic response, hurricane, typhoon, tornado, earthquake, volcanic eruption, nuclear disasters, internal displacement of populations or refugees, etc.). These services may include, but are not limited to:
