PART I - THE SCHEDULE
SECTION B - SUPPLIES OR SERVICES AND PRICES/COSTS
B.1 SUPPLIES/SERVICES
B.1.1 The Contractor shall provide the necessary qualified personnel, facilities, materials, supplies, equipment, and services, except as identified in Section H.3, Government Furnished Property, to perform the inspection, compliance testing and reporting services to determine conformance with the performance requirements of the Federal Motor Vehicle Safety Standard (FMVSS) No. 214, Side Impact Protection.
B.1.2 This is an Indefinite Delivery, Indefinite Quantity (IDIQ) Contract, whereby Firm-Fixed- Price (FFP) Delivery Orders shall be issued in accordance with Federal Acquisition Regulation
(FAR) 16.5.
B.2 MINIMUM/MAXIMUM OBLIGATIONS
B.2.1 Minimum obligation – Upon execution of the IDIQ Contract, the Government hereby orders the supplies and services of the Known Requirements identified in Price Schedule B.3.3 in the amount of $TBD. This amount represents the “minimum obligation of the Government.”
B.2.2 Maximum obligation – The Contractor shall be obligated to provide services and support up to the total not-to-exceed amounts specified in Price Schedules B.3.4 through B.3.8. The overall IDIQ contract ceiling amount is $TBD. The ceiling amount may be increased upon execution of a bilateral modification.
B.3 CONTRACT PRICES
B.3.1 Contract Prices - The following test prices shall apply for payment purposes and shall include all charges, including, if applicable, charges for storage. Prices for the Base Year shall apply to orders placed for Twelve (12) months following the effective date of the contract.
Optional quantity prices shall be effective for those years shown in each of the contract Price Schedules (B.3.4 through B.3.8).
Services shall be billed at rates in effect at the time the order is placed
Upon delivery and acceptance of supplies and services described in Section C, Statement of Work (SOW), the Government will pay to the Contractor the fixed prices set forth in Price Schedules B.3.3 through B.3.8.
Except for the known requirements identified in B.3.3, the Government is not under any obligation to order any additional services under this contract. The Government reserves the right to increase the quantities of testing services provided under this contract up to the “Total Not-To-Exceed” amount specified for each contract period. Optional quantities, if required by the Government, will be exercised in the form of delivery orders.
B.3.2 Contract Line Items - The Price Schedules include the following Contract Line Items described below:
B.3.2.1 FMVSS No. 214P – Rigid Pole Test – Refers to a rigid pole test conducted in accordance with applicable government test procedures using a Part 572 Subpart U or V side impact test dummy positioned in the driver or front outboard passenger seat of the test vehicle.
B.3.2.2 FMVSS No. 214 - MDB Test (1 test dummy) – Refers to a moving deformable barrier (MDB) test conducted in accordance with applicable government test procedures using a Part 572, Subpart U dummy positioned in the driver or front outboard passenger seat of the test vehicle.
B.3.2.3 FMVSS No. 214 – MDB Test (2 test dummies) – Refers to a moving deformable barrier (MDB) test conducted in accordance with applicable government test procedures using a Part 572, Subpart U dummy positioned in the driver or front outboard passenger seat and a Part 572, Subpart V dummy positioned in a rear outboard seat on the second row of the test vehicle.
B.3.2.4 Additional Data Channel – Refers to the per unit cost for installing, collecting and reporting data from one (1) additional dummy data channel or vehicle accelerometer.
B.3.2.5 Post-crash Air Bag Deployment & Restraint Deactivation - Refers to the per unit cost to render all safety restraints inactive by safely deploying all undeployed air bags and seatbelt pretensioners, and, removing at least 1 foot of webbing from each seatbelt.
B.3.2.6 Video Conferencing (Pre-test, impact event and post-test) – Refers to the per unit cost for performing video conferencing of the test as specified in C.5.5.7.
B.3.2.7 Indicant FMVSS No. 305 Test – Refers to the per unit cost to conduct an indicant FMVSS No. 305 test in accordance with OVSC TP-305-02

C.2 BACKGROUND
The United States Department of Transportation’s (DOT) mission is to save lives, prevent injuries, and reduce economic costs associated with motor vehicle crashes. In support of DOT’s mission objectives, the National Highway Traffic Safety Administration (NHTSA) promulgates Federal Motor Vehicle Safety Standards (FMVSS). Manufacturers of motor vehicles and items of motor vehicle equipment are required to certify that each product they sell in the United States meets the requirements of all applicable Federal Motor Vehicle Safety Standards (FMVSS).
Under the compliance test program, the Office of Vehicle Safety Compliance (OVSC) selects vehicles and items of motor vehicle equipment for compliance testing by qualified independent testing laboratories. OVSC relies upon the data and analyses generated by the testing to determine whether vehicle manufacturers’ certifications under FMVSS No. 214, Side Impact Protection, are valid.
C.3 PURPOSE
The Contractor shall furnish qualified technical personnel, facilities and equipment to perform inspection, compliance testing, and reporting services to determine conformance of motor vehicles to the requirements of the Federal Motor Vehicle Safety Standard (FMVSS) No. 214, Side Impact Protection. At the time of Contract award, the Contractor shall have all necessary personnel, facilities and equipment, unless stated otherwise in Section H.3 “Government Furnished Property.” The Contractor shall perform and complete testing and associated inspections within the timeframe specified in the Delivery Schedule, Section F.4.1, and shall submit deliverables as required by the Schedule.
C.4 KICK-OFF MEETING
Within 30 days of the effective date of the contract, the Contractor shall meet with the COR, Contracting Officer (CO), and other OVSC and NHTSA staff. The meeting may take place at the Contractor’s test site or via teleconference at the discretion of the Government. This meeting will be approximately one (1) hour in length and include a brief presentation of the Contractor’s understanding of contract requirements. During the presentation, among other pertinent contract issues, the parties shall discuss basic contract requirements, proposed scheduling of tests and test procedures.
C.5 SPECIFIC CONTRACT REQUIREMENTS
C.5.1 Fully Operational Test Site
The Contractor shall possess and maintain a fully operational test site furnished with qualified technical personnel, facilities, equipment and test devices necessary to perform the inspection, compliance testing and reporting services in accordance with the requirements of FMVSS No. 214, Side Impact Protection, and the terms and conditions of this contract for the full performance period outlined in Section F.2, Period of Performance. The Contractor’s fully operational test site shall be sufficiently staffed to perform and complete testing and associated inspections, and to generate and submit deliverables within the timeframes specified in Section F.4.1, Deliverables/Milestones Schedule.
C.5.2 Test Procedures and Quality Assurance /Quality Control Management Plan
The Contractor shall perform all work in accordance with the requirements of the FMVSS, the OVSC Test Procedures (TPs), and any applicable federal regulations, including the following:

Specific contract requirements supersede all requirements in the Office of Vehicle Safety Compliance Test Procedures (TPs) incorporated by reference in Section J. If there is a conflict between the OVSC TPs and the requirements of the contract, the contract shall prevail.
Detailed “in-house” check-off-sheet style test procedures generated by the Contractor will be referred to hereinafter as “Contractor Test Procedures.” Contractor Test Procedures are subject to the NHTSA Contracting Officer’s Representative (COR) approval.
In addition to meeting the requirements of the test procedures, the Contractor shall follow its internal Quality Assurance/Quality Control (QA/QC) project plan developed specifically for this compliance program. Within four (4) weeks of contract award or before the start of any testing, the Contractor shall provide the COR with an update to its QA/QC project plan that was originally submitted as part of its technical proposal. The updated plan shall reflect any changes made since the initial submission. The Contractor shall submit proposed revisions to its QA/QC project plan to the COR for review and approval before implementing any such revisions.
C.5.3 Government Furnished Property (GFP)
Government Furnished Property refers to property in the possession of, or directly acquired by, the Government and subsequently furnished to the Contractor for performance of this contract (Refer to Section H.3).
The Government shall furnish to the Contractor test vehicles and anthropomorphic test devices described below.
C.5.3.1 Test Vehicles
The Government shall arrange for the delivery of test vehicles to the Contractor’s point of receipt.
The Contractor shall inspect a test vehicle immediately upon delivery and document its condition.
A “Vehicle Condition” form will be supplied to the Contractor by the COR for each vehicle transferred to its facility for testing. The upper half of the form describes the vehicle in detail and is to be completed at the time of vehicle receipt. The lower half provides space for a detailed description of the post-test condition and is to be completed at the conclusion of the test. Vehicle Condition forms must be submitted to the COR in accordance with Section F.4.1, with copies of the Final Test Report. Failure to submit the required vehicle condition form will result in rejection of the Final Test Report.

The Contractor shall be responsible for the storage and security of all test vehicles from receipt until sale or transfer from its facility as instructed by the COR.
The Contractor is responsible to accept test vehicles from automobile dealers/distributors, or vehicle transporters, or including government-contracted transporters.
If a vehicle is delivered by a dealer/distributor or a vehicle transporter (other than a government-contracted transporter), the Contractor shall verify the following:
• Tires and wheel rims are new.
• There are no dents or other interior or exterior flaws in the vehicle body.
• The vehicle has been properly prepared and is in running condition.
• The glove box contains an owner’s manual, warranty document, consumer information, and extra set of keys.
• Proper fuel filler cap is supplied on the vehicle.
• All options listed on the “window sticker” are present on the test vehicle.
• The VIN (vehicle identification number) on the vehicle condition report matches the
VIN on the vehicle.
If a vehicle is delivered by a Government-contracted transporter, the Contractor shall check for damage which may have occurred during transit and verify the VIN on the vehicle condition report matches the VIN on the vehicle.
The Contractor shall notify the COR via email within 24 hours of receipt of a test vehicle.
The Government will determine the sequence in which vehicles are to be tested and the sequence for each specific procedure and will not be bound by any plans, lists, matrices, or other preliminary data submitted to the Contractor prior to delivery of the vehicles. Prior to testing, the Contractor shall provide the test schedule required in Section C.5.5.3. and verify with the COR the proper sequence and procedures for the test vehicles.
The tested vehicles shall be stored in a secure location that provides protection against theft and adverse weather. The Contractor also shall store the vehicle until disposition instructions are provided by the COR.
GFP test vehicle(s) shall not be driven by the Contractor on public roadways, unless written permission is provided by the NHTSA COR.
C.5.3.2 Anthropomorphic Test Devices
The Government shall furnish Part 572 Subpart U and V Anthropomorphic Test Devices (ATDs) (referred to as Side Impact Test Dummies (SIDs)) to the Contractor for exclusive use in the OVSC compliance testing program. The ATDs will be furnished with load cells to measure abdomen and pelvis forces, and chest displacement transducers to measure chest deflections.
The Contractor shall furnish and install accelerometers in the head of the Part 572 Subpart U side impact test dummy and accelerometers in the head and spine of the Part 572 Subpart V side impact test dummy.
The Contractor shall inspect and calibrate each ATD upon delivery and provide results of the inspection to the NHTSA COR in accordance with Section F.4.1. The ATDs shall be stored in a secured room that is kept between 12.8°C and 29.4°C in an upright-seated position with the weight supported by the internal structure of the pelvis. The dummy’s head shall be held upright without supporting the weight of the dummy, by using an eye-bolt that can be secured in the top of the head. The Contractor shall check the dummy for damage when performing subsequent calibrations. The Contractor shall keep a detailed record of calibrations for each ATD, including a list of parts replaced. The Contractor shall send ATDs to the Government for refurbishment as directed by the COR.
C.5.4 Measurement and Test Equipment
The Contractor shall implement and maintain a calibration system in accordance with this Contract and all applicable TPs.
Before the Contractor initiates the safety compliance test program, a test instrumentation calibration system shall be implemented and maintained in accordance with established calibration practices. The calibration system shall be set up and maintained as follows:
• Standards for calibrating the measuring and test equipment shall be stored and used under appropriate environmental conditions to assure their accuracy and stability.
• All measuring instruments and standards shall be calibrated by the Contractor, or a commercial facility, against a higher order standard at intervals not exceeding 12 months for instruments and 12 months for calibration standards. Records, showing the calibration traceability to the National Institute of Standards and Technology (NIST), shall be maintained for all measuring and test equipment.
• All measuring and test equipment and measuring standards shall be labeled with the following information:
(1) Date of calibration
(2) Date of next scheduled calibration
(3) Name of the technician who calibrated the equipment
• A written calibration procedure shall be provided by the Contractor including, as a minimum, the following information for all measurement and test equipment:
(1) Type of equipment, manufacturer, model number, etc.
(2) Measurement range
(3) Accuracy
(4) Calibration interval
(5) Type of standard used to calibrate the equipment (calibration traceability of the standard must be evident)
(6) The actual procedures and forms used to perform the calibrations.
• Records of calibration for all test instrumentation shall be kept by the Contractor in a manner that assures the maintenance of established calibration schedules. All such records shall be readily available for inspection when requested by the COR. The calibration system procedures shall be accepted by the COR before the test program commences.
• Test equipment shall receive a calibration adjustment immediately prior to a test and a calibration check after the test. This check shall be recorded by the test technician(s) and submitted with the final report.
• The COR may require the Contractor to evaluate its data acquisition system to demonstrate ability to meet procedural requirements.
Further guidance is provided in the International Standard ISO 10012-1, “Quality Assurance Requirements for Measuring Equipment” and American National Standard ANSI/NCSL Z540-1, “Calibration Laboratories and Measuring and Test Equipment - General Requirements.”
NOTE:
In the event of a failure of a test item to meet the standard’s minimum performance requirements, as outlined in each TP under Section J (List of Attachments), the COR may require additional calibration checks of critically sensitive test equipment and instrumentation to verify accuracy. The need for additional calibration under these circumstances will be at the COR’s discretion and shall be performed without additional cost to the Government.
C.5.5 Test Requirements
C.5.5.1 Deviation from OVSC Test Procedures
The Contractor shall conduct testing only in accordance with the OVSC Test Procedure and applicable FMVSS or federal regulations and shall notify the COR if any deviation from the OVSC Test Procedure is necessary. The Contractor shall receive approval from the COR prior to initiating any deviations from the OVSC Test Procedure or the approved Contractor Test Procedure. The COR is authorized to approve such deviations as long as that deviation does not affect the prices stated in this contract.
C.5.5.2 Storage of Test Vehicles, Items, Parts, Components, and Equipment
Test items, including vehicles, items, parts, components and equipment, shall be stored by the Contractor at no additional cost to the Government until receipt of disposition instructions from the Government. (Refer to Section H.14.)
C.5.5.3 Test Schedule
The Contractor shall submit a test schedule to the NHTSA COR prior to testing. Tests shall be completed as required in the contract. All testing shall be coordinated to allow monitoring by the COR. (Refer to Section F.9)
C.5.5.4 Test Access
No individuals, other than Contractor personnel directly involved in the compliance testing program, shall be allowed to witness any vehicle compliance test unless specifically authorized by the COR. The Contractor shall secure the test site area immediately before, during and immediately after a test and to shield the test area from public view.
No individuals, other than Contractor personnel directly involved in the compliance testing program, shall be allowed to inspect NHTSA test vehicles or witness vehicle preparation without prior written permission from the COR.
The Contractor shall not have communications with vehicle manufacturers or the general public about compliance testing under this contract. The Contractor shall refer any communication requests from manufacturers or the general public to the COR.
The Contractor shall not release test data without permission of the COR. Please note that the Government considers it a contract breach to discuss or release test data to vehicle manufacturers or the general public without permission of the COR. Accordingly, the Contractor should take all reasonable steps to prevent unauthorized releases of data.
C.5.5.5 Test Report and Other Deliverables
The Contractor shall furnish test reports and deliverables for each test performed in accordance with Sections F.4.1 and F.15. The draft and final reports shall be formatted as specified in F.14 and in the OVSC test procedures.
C5.5.6 Post-crash Air Bag Deployment & Restrain Deactivation
Following completion of testing and prior to disposal of a vehicle, the Contractor shall make all air bags inactive by safely deploying all un-deployed air bags. The Contractor shall also make all seatbelt assemblies inactive by safely deploying all un-deployed pretensioners and removing and discarding a large section of webbing (i.e., 1foot (30.5 cm)) from each seatbelt.
C5.5.7 Video Conferencing
Upon receipt of delivery order from the government, the contractor shall host a video conference on test day that includes live video streaming of the pre-test condition of the test vehicle, the impact event and post-test condition of the test vehicle. The video conference shall be conducted in accordance with TP-214P-01, Appendix C and TP-214D-09, Appendix D for the pole test and moving deformable barrier test, respectively.
C.5.5.8 Indicant FMVSS No. 305
For electric powered vehicles subjected to a MDB impact test, the Contractor shall also conduct a FMVSS No. 305 test in concert with the MDB impact test to verify operation and status of the test vehicle’s high voltage propulsion battery system in accordance with OVSC TP-305-02. The Contractor shall provide a separate test report summarizing the test results as indicated in TP-302- 02 except that the final report cover and test summary page shall indicate that the test is “indicant.” The draft and final test reports shall be submitted at the same time as the draft and final test reports for the MDB test (See Item Nos. 17 and 18 of Section F.4.1.).

SECTION D - PACKAGING AND MARKING
D.1 GENERAL INSTRUCTIONS
Preservation and packaging of all shipments or mailing of all work delivered under this Task Order shall be done in accordance with good commercial practices and to insure acceptance by common carrier (if applicable) and safe transportation at the most economical rate(s).
Except as specifically exempted by the Contracting Officer or COR (TO), all deliverables under this Task Order shall be sent in electronic format, as specified in Section F.
D.2 FORMATTING TEXT AND IMAGE DELIVERABLES
For those deliverables submitted electronically as text or image files, the following specifications apply.
D.2.1 Text
Text files shall be delivered in a format compatible with a current version of Microsoft WORD. Font type of Times New Roman with a font size of twelve (12) shall be used throughout the document. The Contractor shall minimize additional formatting as much as possible, for example, avoid use of columns. Avoid using any design/layout elements except as described in Sections D.2.2, D.2.3, and D.2.4 below.
D.2.2 Charts and Graphs
The Contractor shall develop charts or graphs using a format compatible with a current version of Microsoft EXCEL and shall insert these in the appropriate places within the text document. The Contractor shall also provide the Microsoft EXCEL source file from which charts or graphs were developed.
D.2.3 Illustrations
Illustrations can take two (2) forms:
a. Graphics – The Communication Services Division will develop any needed graphics in collaboration with the COR (TO) and if desired, the COR (TO)’s contractor. The Contractors shall provide only the text files in Word to the COR (TO). In the event that it is necessary for the contractor to develop the graphics, only GPO-approved software such as Adobe Illustrator shall be used.
b. Photographs – When photos are used in Agency publications, the Communication Services Division typically uses existing photos or photos purchased through various sources. In the event that a contractor or other source will supply photos for use in a publication, the photos must be high resolution images preferably at 300 dpi (4” by 5” in size) or higher. They should be submitted electronically in either a tif (usually from a photo that was taken, developed, printed, and then scanned) or a jpg (usually taken using a digital camera and then downloaded onto a computer or disk) file.
NOTE: If a photograph is supplied by an outside source (contractor, grantee, national organization, etc.), a release form must accompany the photo indicating that the human subjects in the photo authorize NHTSA to use the photo in its publications and on its Internet site. In addition, please use caution when allowing contractors to supply photos. Make sure that you know the contractual arrangements they have made with the photographer and the subjects in the photo to ensure that NHTSA has unlimited rights to use the images in print on the web. You must obtain a written statement from your contractor indicating that NHTSA has unlimited, unrestricted use of the photos.
D.2.4 Web-based Images
Occasionally, web-based images may be used in NHTSA publications. The Contractor is cautioned that copyright laws restrict use of many of these images even if this is not expressly indicated at the website. Additionally, the resolution of most web-based images is not high enough to provide a clear image when published. For these reasons, web-based images should not be used.

F.2 PERIOD OF PERFORMANCE
All work and services required hereunder shall not exceed 60 months from the effective date of the contract inclusive of one (1) base year and four (4) option years that may be exercised at the Government’s discretion.

F.5 VEHICLE CONDITION REPORT FORM
The Contractor’s receipt and final inspection of each test vehicle shall be documented by completing the applicable section of the Vehicle Condition Report (See Section 7 - Government Furnished Property of the TP). The COR will provide the Contractor with an electronic copy of the Vehicle Condition Report form at the beginning of the contract period. The form will be in a Microsoft Office compatible file type, such as Microsoft Word or Microsoft Excel. The Contractor shall complete the applicable section of the form documenting the initial condition of the vehicle at the time of its receipt and the condition after the performance of a test. For the initial or pretest condition, the Contractor shall submit the completed portion of the Vehicle Condition Report to the COR within one (1) week after receipt of a test vehicle. For the final or posttest condition, the completed vehicle condition report shall be submitted with the Final Test Report. The completed condition report form shall be included with the CD-ROM, DVD or USB flash drives containing the Final Test Report in PDF format.
F.6 ANTHROPOMORPHIC TEST DEVICE (ATD) INITIAL INSPECTION REPORT
The Contractor shall perform an inspection and calibration upon receipt of each ATD and report the results to the NHTSA COR and ATD Federal Project Manager in accordance with Section F.4.1.
F.7 TEST INSTRUMENTATION/EQUIPMENT CALIBRATION PROCEDURES
Within two (2) weeks of the effective date of the contract, or prior to commencing the first compliance testing under this contract (whichever occurs first), the Contractor shall provide a copy of its calibration procedures. The procedures shall explain how the Contractor implements and maintains its instrumentation calibration system to assure that calibration schedules are met in accordance with applicable Test Procedures. The procedures shall indicate whether the system complies with ISO 10012-1, ANSI/NCSL Z540-1 or other standard criterion and shall address the minimum requirements identified in the TPs. (See C.5.4 for further description of the required procedure.)
F.8 CONTRACTOR’S IN-HOUSE LABORATORY TEST PROCEDURES
Prior to conducting its first test for each type of FMVSS test required under the contract, the Contractor shall provide the COR with a detailed in-house test procedure and detailed check-off lists.
The test procedure shall include a step-by-step procedure to be used for documenting and completing each type of FMVSS test. Each test procedure shall include, as a minimum, the following: general requirements, list of test equipment, photograph requirements, pre- and post-test requirements, test execution process, and data sheets.
OVSC test procedures are available for the Contractor to use as guidance for development of its in-house procedures. The check-off lists shall document the step-by-step test execution process and should be used in conjunction with the test procedure. The test technician shall use check-off lists to document the completion of each step within the test process to ensure every step is completed in the proper sequence as required. Provisions for issuance and control of the Contractor’s test procedures and check-off lists shall be addressed within its QA/QC project plan.
F.9 TESTING SCHEDULE/CONDUCT
Within one (1) week after receipt, inspection and acceptance of the test vehicle, the Contractor shall provide a test schedule to the COR. Within two (2) weeks after receipt of each test vehicle, the Contractor shall begin testing.
In the event the Contractor’s inspection of the test vehicle reveals a discrepancy, such as missing equipment or damage to the test vehicle that would prevent the Contractor from conducting a test, the Contractor shall schedule the test within one (1) week after remedial action has resolved the identified issues.
The Contractor shall not deviate from the test schedule unless authorized in writing by COR.
F.10 TEST DATA SUMMARY OF RESULTS AND IMPACT EVENT PHOTOGRAPH
Within 24 hours of the test, the Contractor shall provide to the NHTSA COR, a data summary of test results which includes the impact speed, impact point and dummy injury data results. In addition, the Contractor shall provide a digital photograph of the impact event in JPEG format.
F.11 TEST FAILURE REPORTS
Within 24 hours of a test failure, the Contractor shall provide summary notification of that failure to the COR. This summary notification may be transmitted via telephone.
Within 48 hours of a test failure, the Contractor shall provide the COR with a written notification that includes the following information: FMVSS test type, test date, laboratory, contract number, project engineer’s name, test failure description, actual data, e.g., charts, any data relevant to the failure, including video and/or photographs, charts (including plots and values), etc., section of the standard failed, date of COR notification, and the name of the individual that provided COR notification. A sample form will be provided upon request by the COR.
The COR may require the Contractor to provide additional information related to the circumstances of a test failure. This information may include anything deemed relevant by the Government in understanding the nature of the test failure that occurred. Under this contract, the Contractor shall be obligated to provide this information within the time requested by the COR. Failure to provide the information within the time requested will be treated as a breach of contract performance.
F.12 MONTHLY STATUS REPORTS
Monthly status reports shall be submitted to the NHTSA COR and the Contracting Officer, indicating the status of all testing services, test reports, GFP received for the calendar month, and shall be submitted not later than the 15th day of each month following the month being reported.
The monthly status report shall indicate the contract number and the time covered. The Contractor shall continue to submit monthly status reports until all final test reports are accepted by the
Government. Specifically, Monthly Status Reports shall include the following:
• Vehicle make, model, model year, and, the vehicle number assigned by OVSC (NHTSA number).
• Date of vehicle receipt, date of vehicle condition report.
• Number of tests, inspections, and reports scheduled for the reporting period and completed during the reporting period;
• Schedule for tests (that includes the scheduled test date, actual test date, pass/fail, draft report submission date, final report submission date, invoice submission date, and date of vehicle transfer (if applicable)), inspections, and reports for the next reporting period;
• Brief description of test failures which occurred during the reporting period;
• Problems or delays that the Contractor experienced during the reporting period which are related to the supplies and services required under this contract;
• Specific action which the Contractor proposes to correct problems or delays identified.
F.13 ANNUAL PHYSICAL INVENTORY REPORT
The Contractor shall submit a copy of its Annual Physical Inventory Report – Contractor Inventory Schedule (HS Form 324) in accordance with Section F.4.1.
F.14 DRAFT AND FINAL TEST REPORTS AND TEST VIDEO
Draft test reports and all test video shall be submitted to the designated COR for review and acceptance within fourteen (14) calendar days of completion of all phases of compliance testing and inspection of each test item. Test reports shall be prepared in accordance with instructions contained in the OVSC Laboratory Test Procedure. Reports that do not meet these requirements will be returned to the Contractor. NHTSA may withhold payment of a Contractor’s invoice until the final test report is accepted by the COR. If a final test report is returned to the Contractor for technical or format corrections, the report date shall be changed to reflect the date of resubmission.
The first four pages of the test report shall be in the following format:

A heavy paperback cover (or transparency) shall be provided for the protection of the hardcopy final report. A jewel case shall be provided for the protection of the CD-ROM or DVD. USB flash drives may also be provided, as mutually agreed to by the Contractor and the COR. The information required on the cover is as follows:
