About this file

This is a Schedule B Pricing document for Request for Proposal No. 260000002381, issued by the Michigan Department of Health and Human Services (MDHHS) for Family Preservation – Family Assistance Program Services in Alcona and Iosco Counties. The RFP seeks qualified contractors to provide two service components: Direct Family Assistance and Parenting Time Support, both billed on an hourly basis. The contract term is three years, running from October 1, 2026 through September 30, 2029, with an annual award of $50,000.00 for each year. Bidders must submit pricing schedules in modifiable format (Microsoft Word or Excel) as separate documents from other proposal materials, with failure to complete the pricing schedule as requested resulting in proposal disqualification.

Bidders must provide unit rates for each service that encompass all direct and indirect costs, including mandated insurance. An annual allocation of $10,000.00 for Direct Family Assistance transportation expenses has been identified separately and must not be factored into the unit rate calculation, though it must be included in the maximum award amount. Total proposed costs across all units and actual cost components cannot exceed the $50,000.00 annual maximum award. Travel reimbursement for mileage, meals, and lodging is limited to the lesser of the state premium rate or the contractor's usual employee reimbursement rate and is permissible for client transportation, travel to client homes, court hearings at MDHHS worker request, and advocacy-related site visits, with mileage calculated from the contractor's home or normal place of business, whichever is closer to the appointment location.
