About this file

This is a pricing schedule document for Request for Proposal No. 260000000604 issued by the State of Michigan for Temporary Medical Staffing Services. The RFP seeks contractors to provide temporary medical staffing across a comprehensive range of healthcare positions, including physicians, nurses, dentists, pharmacists, therapists, social workers, counselors, and various support personnel. Contractors must submit pricing proposals in modifiable formats such as Microsoft Word or Excel, with the option to include additional non-modifiable formats such as PDF. Failure to complete the pricing schedule as requested may result in proposal disqualification. The pricing schedule requires contractors to propose an hourly rate of pay for employees placed with the State, a billable rate that the State will be charged, and an overall markup percentage that covers all expenses associated with temporary staffing placements.

Price proposals must include all costs, including one-time or setup charges, fees, and any potential charges such as shipping, handling, and palletizing. Contractors are encouraged to offer quick payment terms with specified discount percentages if invoices are paid within a designated number of days of receipt, excluding processing time for payment to the contractor's financial institution. The markup percentage must be consistent across all positions and must cover all expenses associated with placing temporary medical staff with the State. Additionally, the pricing schedule includes a line item for MDOC Computer-Based Training at a fixed rate of $10.00 per person per module with no contractor markup applied. By submitting a proposal, contractors certify that prices were developed independently and without consultation or agreement with other contractors.
