About this file

This is a pricing schedule document for Request for Proposal No. 260000000604, issued for Temporary Medical Staffing Services. The RFP requires contractors to provide comprehensive pricing for a range of medical and clinical positions, including physicians, nurses, therapists, technicians, and support staff across multiple specialties such as dentistry, dermatology, psychiatry, and social work. Contractors must submit pricing schedules in modifiable formats such as Microsoft Word or Excel, with the option to provide an additional non-modifiable PDF version. The document does not specify response deadlines, site visits, bidders' conferences, award dates, or contract terms, as these details are contained in other sections of the RFP not included in this file.

The pricing structure requires contractors to submit three distinct rate components for each position: the hourly rate (amount paid directly to placed employees), the billable rate (amount charged to the State), and the markup percentage (covering all placement-related expenses). Price proposals must include all costs such as setup charges, fees, shipping, and handling. Contractors are encouraged to offer quick payment discounts for invoices paid within a specified number of days. The document includes a training component, with MDOC computer-based training set at a fixed rate of $10.00 per person per module without contractor markup. By submitting proposals, contractors certify that pricing was developed independently without consultation or agreement with other contractors.
