About this file
This document is a Request for Proposal (RFP) No. 250000002916 issued by the Michigan Department of Health and Human Services (MDHHS) for Family Preservation - Family Assistance Program Services. The contract will span three years, from November 1, 2025, to September 30, 2028, with an estimated annual allocation of $70,000.00 per year. The RFP seeks providers to deliver two primary service components: Direct Family Assistance Services and Parenting Time Support Services, with contractors expected to provide hourly-based services. The proposal requires detailed unit rate pricing and includes specific guidelines for service delivery and cost calculations.
The pricing structure mandates that bidders submit comprehensive unit rates that encompass all direct and indirect costs, including mandated insurance expenses. Travel reimbursement will follow state rates, with specific guidelines for mileage billing related to client services. Contractors must ensure their total proposed unit rates and actual cost components do not exceed the maximum annual award amount. The proposal emphasizes independent pricing determination and requires submission of pricing schedules in modifiable formats, with potential disqualification for incomplete submissions. Notably, transportation costs are separately allocated at $17,000 annually and are not to be calculated into the proposed unit rates.
