About this file
This document is a Service Level Agreements (SLA) schedule for a contract between the Michigan Department of Corrections (MDOC) and an unnamed contractor, covering various performance metrics for correctional services. The contract involves eight primary performance metrics that assess the contractor's compliance in areas such as chart review audits, staff qualifications, staffing levels, Law Enforcement Information Network (LEIN) clearance, billing, training, and responsiveness to corrective action plans.
The service level agreement establishes specific financial penalties for non-compliance across each metric, with service credits ranging from $250 to $5,000 depending on the severity and frequency of the issue. Credits can be applied toward future payments or paid directly to the State of Michigan within 10 calendar days of demand. The contract emphasizes 100% compliance in most metrics, with some initial metrics allowing for slightly lower compliance rates (such as 85% in the first contract year for chart review audits). The document provides detailed methodologies for assessing compliance, including random sampling, license verification, staffing level reviews, and annual training audits.
