About this file

This is a Cost Proposal template (Exhibit B-1) for RFP 24-10536 issued by the California Department of Public Health (CDPH). The contract period spans from March 3, 2025, to June 30, 2028, divided into four fiscal years. The scope includes two main goals: planning and facilitating a Community Action to Fight Asthma (CAFA) Asthma Summit, and leading coordination of the California Healthy Housing Coalition while facilitating connections to asthma activities led by CDPH.

The total contract allocation is $149,999.99, and any proposals exceeding this amount will be deemed nonresponsive. The contract is deliverables-based, and rates must be fully burdened to include operating expenses, labor, equipment costs, supplies, overhead, annual inflation costs/rate adjustments, profit margin, taxes, and shipping. The cost proposal requires contractors to break down costs by fiscal year for each of the two main goals, with signature required from an authorized representative to be considered responsive.
