About this file

This is a Cost Proposal Form (Submittal Form G) for State of Alaska RFP 2025-1600-0123. The form provides detailed instructions for submitting cost proposals related to Medicaid-related consulting services, including financial and actuarial analysis, policy consulting, program support, federal compliance, section 1115 demonstrations, Advanced Planning Document support, coordinated care demonstration projects, and project management services.

The pricing structure requires vendors to submit two tables: Table 1 for Personnel Roster & Hourly Rates and Table 2 for Example Project Costs. Personnel are categorized into four levels based on experience: Level 1 (15+ years), Level 2 (10-15 years), Level 3 (5-10 years), and Level 4 (less than 5 years). Only one hourly rate may be proposed per personnel level. The Example Project Costs table must align with the project plan described in Submittal Form D and reflect activities from Section 3.01 of the RFP. While the Project Total Amount will be used for evaluation purposes, it will not be binding under the final contract.

RFP 2025-1600-0123

SUBMITTAL FORM G – Cost Proposal Guidelines Offerors must follow the format below and provide detailed responses in Table 1 (Personnel Roster & Hourly Rates) and Table 2 (Example Project Costs). The Personnel Roster (Table 1) must reflect the offeror’s capability to provide the expertise required to staff the types of projects described in RFP Section 3.01 (Scope of Work), including financial and actuarial analysis, policy consulting and program support, federal compliance support, section 1115 Medicaid demonstrations, Advanced Planning Document (APD) support, coordinated care demonstration projects support, and project management and stakeholder engagement. The Example Project Costs (Table 2) must reflect the example project management plan described in Submittal Form D and must represent the offeror’s approach to efficiently leveraging personnel resources to carry out project plans.

Personnel Roster & Hourly Rates Instructions:

1. List all personnel titles that may work on the contract and assign each title to the designated personnel levels defined below. Add rows within the given personnel levels as needed.

2. Provide an hourly rate for each level. Only one hourly rate may be proposed per personnel level, even if multiple personnel titles are included in the level.

3. Ensure hourly rates align with the qualifications and expertise outlined for each level.
