Federal Emergency Management Agency Department of Homeland Security FEMA Region VI Austin Texas Recovery Office on behalf of FEMA Region VI

REQUEST FOR INFORMATION – PERSONAL PROTECTIVE EQUIPMENT (PPE)

FACE MASKS, FACE SHIELDS, GOWN/OVERALLS, GLOVES

***This is potentially a FAR Part 18 Emergency Acquisitions or FAR 6.302-2 Unusual and compelling urgency requirement due to the novel Coronavirus (COVID-19) pandemic where a presidential Emergency Declaration was made, and potential Major Disaster Declarations may result. As such, the entirety of this Request for Information (RFI) is subject to change. Multiple authorities and/or singular authorities are subject to change and it is incumbent on all vendors to monitor this RFI continuously.

The U.S. Government / Federal Emergency Management Agency (FEMA) is seeking information and sources for immediate delivery of medical supplies and personal protective equipment (PPE). This is not an order for supplies.

The U.S. Government is interested in establishing multiple awards for immediate purchases of Face Masks, Gowns / Overalls, Gloves, and Face Shields / Eye Protection used in healthcare settings as described below. This will enable the United States to ensure sufficient availability of such items during national emergencies and/or pandemic events. The Government is concerned about the risks of disruption of any manufacturing facilities or supply chains located outside of the United States. Thus, offerors are advised to pay special attention to the instructions regarding manufacturing capability and/or supply chains located outside of the United States as well as the evaluation criteria related to manufacturing capability and foreign supply chains. The Government is concerned about the risks related to excessive pricing of critical PPE and intends only to acquire PPE at prices that it can determine are fair and reasonable. Thus, offerors are advised to pay special attention to the instructions regarding pricing.

Description/Tentative Items:

The Federal Emergency Management Agency (FEMA), Region VI is seeking information related to how interested contractors could provide the follow item(s) (Please note that this list is subject to change):

1. FACE MASKS

The following is a summary to provide clarification about the different types of acceptable Face Masks for facial protection that may be proposed as part of the requirement:

a. Surgical masks are fluid-resistant, disposable, and loose-fitting protection devices that create a physical barrier between the mouth and nose of the wearer and the immediate environment. FDA reviews and clears surgical masks under 21 CFR 878.4040 as Class II medical devices, which may be labeled as surgical, isolation, dental, or medical procedure masks. They may come with or without a face shield. These are often referred to as face masks, although not all face masks are regulated as surgical masks.

b. Face masks are disposable loose-fitting protection devices that create a physical barrier between the mouth and the nose of the wearer and the immediate environment. Face masks that are not cleared or approved by FDA preferably should demonstrate conformance to the following performance requirements through either a 1) Manufacturers declaration of conformity or through verification of testing by a 2) ISO 17025 accredited laboratory declaration of conformity to the following performance requirements:

c. Fluid resistance: Fluid resistance is met if the mask passes the test method below, or comparable test method, at any specific velocity as identified in the labeling. The mask shall be tested on a pass/fail basis at velocities specified in the standard.

ASTM F 1862: Standard Test Method for Resistance of Surgical Mask to Penetration by Synthetic Blood Viral or bacterial filtration performance (e.g., filters 95% of bacteria) Filtration testing (e.g., ASTM F2101–01 (Bacterial Filtration Efficiency) and ASTM F2299 (Particulate Filtration Efficiency)

d. Biocompatibility: Appropriate analysis and testing (such as that outlined in the ISO-10993-1 standard, “Biological Evaluation of Medical Devices Part 1: Evaluation and Testing within a Risk Management Process.”) must demonstrate the biocompatibility of the mask. Tests selected must be appropriate for expected duration and level of contact with the device (e.g., limited contact devices including skin). Masks should be tested on a pass/fail basis with respect to cytotoxicity, sensitization, and irritation.

2. GLOVES

Protection of healthcare workers and first responders from transmission of pathogens is essential to maintaining resilience of the U.S. healthcare system. Non-Sterile, Powder-Free, Patient Examination Gloves are one part of an infection control strategy and are examples of personal protective equipment (PPE) that is a disposable device intended for medical purposes that is worn on the examiner’s hand or finger to prevent contamination between patient and examiner. The appropriate use of PPE is an important component of the overall strategy of certain infection control guidelines.

The following is a list to provide clarification about the different types of acceptable Non-Sterile, Powder-Free, Patient Examination Gloves to protect against contamination that may be proposed as part of the requirement:

Patient Examination gloves are examples of personal protective equipment that are used to protect the wearer and/or the patient from the spread of infection or illness during medical procedures and examinations. 3. FACE SHIELDS / EYE PROTECTION

Protection of healthcare workers and first responders from transmission of pathogens is essential to maintaining resilience of the U.S. healthcare system. Eye protection is one part of an infection control strategy and is an example of personal protective equipment (PPE) that are devices used to help protect health care workers from splashes, sprays, and splatters of body fluids and infectious material in the air. Eye protection is designed to either cover the eyes only or to cover both the eyes and entire face. The appropriate use of PPE is an important component of the overall strategy of certain infection control guidelines.

Eye protection is an example of personal protective equipment that, depending on the type of device, can protect the eyes, face or associated mucous membranes from splashes, sprays, and airborne body fluids. Goggles provide direct eye protection for healthcare workers, whereas face shields offer a protective barrier to the entire facial area, eyes, nose and mouth. The choice of Eye protection is dependent on the level of risk of contamination.

Eye and face protection are manufactured in accordance with the American National Standards Institute (ANSI) International Safety Equipment Association (ISEA) Z.87.1-2010 standard. The following is a list of acceptable protective Eye protection based on their intended use and risk of contamination that may be proposed as part of the requirement:

4. GOWNS/COVERALLS

A. Gowns are examples of personal protective equipment that are intended to promote infection control practices and help protect patients and healthcare workers against microorganisms and body fluids during routine care, examinations, and medical procedures. The choice of gown is dependent on the level of risk of contamination. The American National Standards Institute/Association for the Advancement of Medical Instrumentation (ANSI/AAMI) PB70 establishes a system of classification for surgical and isolation gowns used in healthcare facilities, based on their liquid barrier performance. The ANSI/AAMI PB70 standard includes four standard tests to evaluate the barrier effectiveness of surgical gowns, isolation gowns, and surgical drapes. Based on the results of these standardized tests, four levels of barrier performance are defined, with Level 1 being the lowest level of protection, and Level 4 being the highest level of protection. ANSI/AAMI PB70 levels are as follows:

Level 1: Minimal risk, to be used, for example, during basic care, standard isolation, cover gown for visitors, or in a standard medical unit Level 2: Low risk, to be used, for example, during blood draw, suturing, in the Intensive Care Unit (ICU), or a pathology lab Level 3: Moderate risk, to be used, for example, during arterial blood draw, inserting an Intravenous (IV) line, in the Emergency Room, or for trauma cases Level 4: High risk, to be used, for example, during long, fluid intense procedures, surgery, when pathogen resistance is needed, or infectious diseases are suspected (non-airborne)

The following is a list of acceptable protective gowns based on their intended use and risk of contamination that may be proposed as part of the requirement:

i. Isolation gowns – disposable, typically non-sterile, used to protect health care personnel and patients from the transfer of microorganisms and body fluids in patient isolation situations, used for routine patient care in healthcare settings. Isolation gowns are not for use in surgical settings.

ii. Low risk of contamination – isolation gowns that claim minimal or low levels of barrier protection; ANSI/AAMI PB70 Level 1 or Level 2. These gowns should not be worn during surgical or invasive procedures, or for medium to high risk contamination patients care activities.

iii. Medium to high risk of contamination – isolation gowns that claim moderate to high barrier protection; ANSI/AAMI PB70 Level 3.

B) Surgical gowns – sterile, disposable gowns, devices that are intended to be worn by operating room personnel during surgical procedures to protect both the surgical patient and the operating room personnel from the transfer of microorganisms, body fluids, and particulate matter.

Surgical gowns are regulated by the Food and Drug Administration (FDA) under 21 CFR 878-4040 as a Class II medical device.

Low risk of contamination – surgical gowns that claim minimal or low levels of barrier protection; ANSI/AAMI PB70 Level 1 or Level 2.

Medium to high risk of contamination – surgical gowns that claim moderate to high barrier protection; ANSI/AAMI PB70 Level 3 and Level 4.
