STATEMENT OF WORK

General: GPS Units Compatible with Geotab Telematics

1. Description of Work: This statement of work defines the effort required for VA Texas Valley Coastal Bend Healthcare System (VATVCBHCS), Logistics Fleet services to procure two GPS telematic units compatible with Geotab monitoring system, brand name or equal to Geotab GO9 — Expandable Telematics Device (P/N: GO9-FTEATTF). Harlingen is the location requesting in the Logistics Fleet Department to facilitate the monitoring of the Mobile Clinics that provide care to veterans who cannot travel to and from normal clinics. Proposed 2 GPS units with harnesses, installation, and configuration. Monthly monitoring will be required.

2. Place of Delivery - The vendor shall deliver the described herein to:

3. Period of Performance: Base Plus Four (4) Year Option with monthly service plan, including transmission.

Hours of Performance: Work shall occur Monday – Friday during normal duty hours unless authorized by Logistics/ Mobile Medical Unit (MMU) Manager.

Work will be coordinated between VA POC and contractor during kick off meeting.

4. Product Requirements for GPS Equipment, Installation, Configuration and

Services (2 Each)

• GPS communicating modules no larger than 6”x3” to connect to the OBD ports on 18-wheeler drive units.

• 1st GPS module, wiring, and harnesses have to connect to OBD port in Freightliner

• 2nd GPS module, wiring, and harnesses have to connect to OBD port in Freightliner

• GPS unit along with wiring harnesses/cables, need to be the correct length to be secured/concealed under dash or steering column, and out of the way of vehicle driver.

• GPS units need monthly service with real time active tracking of vehicles. GPS units MUST be compatible and be able to communicate with the Geotab fleet management program and system brand name or equal to Geotab GO9 — Expandable Telematics Device (P/N: GO9-FTEATTF).

• Equipment will need professional installation and configuration by vendor 60 days after award.

5. Documentation: Contractor shall furnish a delivery manifest once all equipment has been delivered. Payment will not be processed until all equipment has been received, installed, and tested by Fleet Manager. A service report is required and shall contain, at a minimum, the following information:

• Date and time of contractor’s arrival on station.

• Complete list of instruments and consumables delivered and installed.

The service report shall itemize every item in the specification. Each item shall state a complete description of all work performed concerning the items.

6. Records Manager: There will be no federal records created, maintained, used, or dispositioned with this contract: regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.]
