About this file

This is Addendum No. 1 for the DPS P12 Driveway Repairs project (DAS RFB927600-01), issued by the State of Iowa's Department of Administrative Services on 02/21/2023. The project involves driveway and parking lot repairs at the Department of Public Safety Post 12 located in Stockton, Iowa, with two bid packages: the primary driveway repair and an alternate lower level parking lot replacement. The bid was scheduled for submission on 02/23/2023 at 2:00 PM, with a tentative Notice of Intent to Award on 03/06/2023, project commencement on 04/03/2023, and substantial completion targeted for 10/16/2023. The addendum includes pre-bid meeting minutes from 02/08/2023, a Q&A section addressing contractor inquiries, and an architectural product substitution approval for a Class A Roadstone aggregate.

Key administrative details include electronic bid submission through the IMPACS Electronic Procurement System, requiring contractor registration, and a 5% bid security. The project will utilize Procore for project management at no cost to contractors, and contractors must provide a project-specific Certificate of Insurance, Performance and Payment bonds, and a list of subcontractors within 48 hours of bid opening. The contract allows flexibility in project phasing, with the requirement that at least one parking lot remains accessible throughout the project. Contractors will be responsible for preparing as-built drawings, and the project will follow a modified ConsensusDocs 802 agreement. Work hours are set from 7:00 AM to 9:00 PM, with specific site rules prohibiting smoking and requiring daily logs and site cleanup.
