About this file

This is a Responsibility Inquiry attachment form used by the State of Oregon for evaluating applicant qualifications prior to contract award and execution for the Security Patrol and Response Services solicitation (PCS24-007RFP) issued by Portland Community College. The form requires applicants to demonstrate capability to meet all contractual responsibilities by providing detailed documentation of financial resources, materials, equipment, facilities, and personnel expertise. A non-mandatory virtual pre-proposal conference is scheduled for April 3, 2024, with proposals due by April 25, 2024. The procurement covers security services across Portland Community College's campuses located in Multnomah County and the City of Portland, Oregon. Applicants must substantiate their experience by documenting contracts of similar nature completed within the last five years, including performance within allocated time and budget parameters.

The responsibility determination process requires applicants to disclose criminal or civil charges, contract terminations for default, lawsuits, outstanding judgments, financial distress, bankruptcy filings, and licensing compliance within specified timeframes—primarily the last three to five years. Applicants must confirm possession of all required licenses, insurance, and registrations to do business in Oregon. A Pay Equity Certificate issued by the Department of Administrative Services is required if the applicant employs 50 or more full-time workers and the contract price exceeds $500,000, with exemptions for certain professional services. The form mandates authorized representative certification that all responses are complete, accurate, and not misleading, with the agency retaining discretion to request additional documentation including profit-and-loss history, balance statements, cash flow information, credit information, bonding capacity, and performance records under previous contracts. Failure to promptly provide requested information or clearly demonstrate responsibility may result in a non-responsibility finding and rejection.
