Discrepancy Form

Subject: Rework for PR # FD20202300248

PR #: FD20202300248

Amendment #:

Buyer Name: Logan Kashanipour

Item/Program Manager: Ashley Underwood Organization: 415 SCMS/GUMAB Phone: 801-777-6185

Exception Reason: 72

Action Taken to Resolve Discrepancy (attach relevant documentation):

The 762 lists 3 approved sources, one small business and one large business. The third source Photonics, hasn’t had an active SAM registration since 2005. I also tried to call all of the numbers listed in SAM for this contractor and there is no answer. The market research also says the engineer was unable to get into contact with someone from Photonics. I spoke with Kaitlin and we felt that Photonics should just be removed from the 762 as an approved source.

I need an updated market research citing if each of the approved sources are a small or large business under the NAICS code 811219 or 811210.

Also the latest contract (FA825022P0071) for this NSN was not mentioned on the 762 or market research.

Action Necessary to make PR Actionable and who is responsible (PR or Buyer) and if PR, then explain what you need:

Need updated market research and 762.
