About this file
This is a Response to Offeror Questions document for the South Dakota Department of Health's Office of Chronic Disease Prevention and Health Promotion, addressing inquiries related to RFP #25-0904006-012 for Worksite Wellness and Chronic Disease Prevention services. The RFP seeks a qualified contractor to provide leadership, technical assistance, education, and resources to support statewide worksite wellness and chronic disease prevention initiatives. The contractor will support approximately 1.0 FTE of personnel and maintain a primary office location in Pierre, South Dakota, though alternative locations are acceptable if travel requirements can be met. Key deliverables include coordinating with state health program coordinators, developing electronic newsletters, planning educational webinars, recruiting worksites to adopt evidence-based strategies, and submitting monthly progress reports. The contractor is expected to travel biannually to Pierre for in-person meetings with the designated point of contact and attend additional Office of Chronic Disease Prevention and Health Promotion meetings throughout the year. Travel is anticipated statewide as needed to support worksites across communities. Proposals were due by February 9, 2026, at 5:00 PM Central Standard Time, with an anticipated award decision on March 9, 2026. The initial contract term runs from June 1, 2026, through May 31, 2027, with options for up to four additional one-year renewal periods extending through May 31, 2031.
An incumbent contractor has been providing these services for over ten years and will continue through the transition to the new awardee. Targeted worksites include any public or private business, including state agencies, local government, community organizations, and private companies. The contractor's role involves supporting program implementation in multiple capacities: assisting specialty program coordinators with delivering activities to worksites and connecting worksites to existing evidence-based programs, providers, and resources. The contractor is not expected to create and deliver all programs independently but rather leverage existing resources while providing direct support when needed. Performance measures and baseline data will be provided at contract commencement to support meaningful measurement of progress. The contractor will track basic participation and implementation data, including worksite participation numbers and employee reach when feasible, and may document success stories and program deliverables. The South Dakota Department of Health will not provide technology platforms, laptops, or other supplies; the awarded contractor must furnish all necessary office equipment and supplies, with these costs included in the proposed budget. Indirect costs are limited to a maximum federally approved rate of 6.3%.

WORKSITE WELLNESS & CHRONIC DISEASE PREVENTION
RFP #: 25-0904006-012
Responses to Offeror Questions:
1. How many full-time personnel are expected to properly support the SD DOH and the 4 specialty programs?
· Up to 1.0 FTE is anticipated to be sufficient to meet the requirements for supporting the programs.
1. Is Pierre, SD the expected primary office location?
· Pierre, SD is the primary location of the program; however, the awarded contractor may be located elsewhere, provided they can meet travel requirements, including occasional travel to worksites, in-person meetings with the point of contact twice a year, and participation in other Office of Chronic Disease Prevention & Health Promotion meetings throughout the year.
1. Will the contractor have a single point of contact or multiple?
· The contractor will have one designated point of contact, physically located in Pierre, South Dakota
1. What is the expected travel frequency/cadence to fulfill contract requirements?
· Travel is expected to include biannual trips to Pierre, SD to meet with the point of contact, as well as attendance at other Office of Chronic Disease Prevention & Health Promotion meetings as needed throughout the year.
1. What are the other cities and locations throughout the state the contractor is expected to travel?
· In addition to Pierre, the contractor should be prepared to travel to communities statewide as needed.
1. Is there an incumbent currently providing these or similar services to SD DOH? If so, how long has the incumbent been supporting this program?
· Yes. An incumbent contractor has been providing these services for over 10 years, supporting the program throughout that time.
1. The RFP references supporting, recruiting, and engaging “worksites statewide”. What are the types of organizations included in the targeted worksites? (e.g., only state department worksites, private company worksites, state departments and private worksites, etc.)
· Targeted worksites include any public or private business, including state agencies, local government, community organizations, or private companies.
1. Will there be a single integrated work plan or separate deliverables for each specialty program area?
· There will be a single, integrated work plan that encompasses all specialty program areas. This unified work plan will outline coordinated activities, timelines, and deliverables across programs, ensuring alignment while still addressing the specific requirements of each specialty area.
1. What is the contractor's role in implementation of programs? Is the expectation to create and deliver all programs? Or will they primarily be connecting worksites to existing providers and resources?
· The contractor will support program implementation in multiple ways. They may assist each specialty program coordinator with delivering activities to worksites and will also connect worksites to existing evidence-based programs, providers, and resources as appropriate. The contractor is not expected to create and deliver all programs, but rather to leverage existing resources while providing direct support when needed.
1. What specific performance measures will be used to evaluate the contractor’s success under this contract? If baseline data exists, will it be provided to allow meaningful measurement of improvement?
· Performance measures have been identified and will be shared with the awarded organization at the start of the contract period. Any available baseline data will also be provided at that time to support meaningful measurement of progress and outcomes.
1. What level of data tracking is required for worksites and does SD DOH have targets for things like enrollment, attendance, outcome, follow-up, etc.?
· The contractor will be required to track basic participation and implementation data, including the number of worksites participating in programs delivered and, when feasible, the number of employees reached. The contractor may also be asked to document success stories and program deliverables or outcomes. Evaluation expectations and reporting requirements will be clarified at the start of the contract period.
1. Will SD DOH provide any technology platforms to support this work such as a wellness portal, learning management system, CRM system, data collection, or other reporting tools?
· At this time we don’t plan to provide any technology platforms.
1. Will SD DOH provide laptops or any other supplies for contractors? Or will the contractor need to include all office equipment in their supply costs?
· SD DOH will not provide laptops or other supplies. The awarded contractor will be responsible for furnishing their own laptops and any other necessary supplies, and these costs should be included in the contractor’s proposed budget.
