Request for Information

The Department of Veterans Affairs is conducting Market research for Safety Learning System (SLS) software and Support Services for the requirement listed below. This is a Request for Information (RFI) only. This RFI is for planning purposes only and shall not be considered a Request for Quotation or a Request for Proposal. Additionally, there is no obligation on the part of the Government to acquire any products described in this RFI. Your response to this RFI will be treated only as information for the Government to consider. You will not be entitled to payment for direct or indirect costs that you incur in responding to this RFI. This RFI does not constitute a solicitation for quotes/proposals or the authority to enter negotiations to award a contract. No funds have been authorized, appropriated, or received for this effort. Interested parties are responsible for adequately marking proprietary, restricted, or competition sensitive information contained in their response. The Government does not intend to pay for the information submitted in response to this RFI.

It is requested that all companies interested in participating in this effort please note their interest and provide indication of their respective capabilities to perform the effort described below.

Requirement The Department of Veterans Affairs (VA) Office of Information & Technology, Office of Systems Engineering, requires the Safety Learning System (SLS) to support the Department of Planning, Performance, and Development for a current High Reliability Organization (HAO) collaborative project. The current HAO project is looking at the organization's Standardized Mortality Ratio (SMR) and how to improve. The SLS program will analyze all the relative information and give the facility different areas to concentrate on to improve the overall scores in Strategic Analytics for Improvement and Learning (SAIL). VA's requirements consist of the SLS which is web-based software, Doctors, and nurses with clinical knowledge for the review of 100 case files for the baseline study into the SMR and SMR30 measures for the SAIL metrics. The focus is to use actionable data and information to stimulate meaningful cross-disciplinary and cross-departmental dialogue to cut through cultural and system barriers. This facilitates the pinpointing of common causes of patient harm in their systems and processes, provides the technology solution to monitor action and inactions that lead to this harm, and develop processes to address these failures of care delivery. A supported software solution will provide a workflow process to support, facilitate & manage the clinically validated process for case reviews without protected health information.

Safety Learning System Software is used to focus actionable data and information to stimulate meaningful cross-disciplinary and cross-departmental dialogue to cut through cultural and system barriers. Type of Contract: Firm Fixed Price (FFP)

Period of Performance: The Base Period of Performance is one (1) twelve (12) month base and four (4) twelve-month option periods if exercised.

All contractor resources shall be legally authorized to work in the United States.

How to Respond:

Be advised that set-aside decisions may be made based on the information provided in response to this RFI. Responses should be as complete and informative as possible. It is requested that interested offerors respond to the following questions:

a. Provide a brief summary of your technical capability to meet the requirements.

b. Please provide any existing contract vehicles (GSA FSS, GWAC, etc) you currently hold for which this requirement would be within scope.

c. Name of the firm, point of contact, phone number, email address, DUNS number, CAGE code, a statement regarding small business status (including small business type (s)/certifications(s) such as SDB, 8(a), HUBZone, SDVOSB, WOSB, etc.) under the corresponding NAICS code.
