2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

This requirement is for a VuMAX HD Ophthalmic Ultrasound System. This is a diagnostic device that measures the length of the eye for intraocular lens implant calculations for cataract and other surgeries. It can also scan the back of the for retinal detachments and intraocular tumors. This requirement is needed to address the needs of a multi-subspecialty ophthalmology practice, Refractive Surgery Clinic, and Ophthalmic Physicians training program that supports the Veterans in the Muskogee VA Health Care System.

This request for a brand name purchase is intended to result in the award of a Firm Fixed-Price contract for the procurement of VuMAX HD Ophthalmic Ultrasound System manufactured by Sonomed, Inc .

The estimated value of the proposed action is $119,640.00.

The estimated delivery date is 60 days ARO.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

The ophthalmology department has used the VuMax HD Ophthalmic Ultrasound system for the past 5 years, this equipment must be compatible with existing equipment. Their existing equipment (VuMax Ophthalmic) has provided excellent results. The staff is familiar with the equipment brand and would recommend using the same brand to maintain consistency. Switching brands may lead to incorrect intraocular lens calculations which could lead to unnecessary surgery. The ophthalmic surgeons have preprogrammed their unique characteristics in data measurement analysis in the current Sonomed, Inc. machines and an attempt to abruptly change the data measurement analysis may create inaccurate measurements, alter post-operative results and cause unnecessary surgeries.

The same brand has also provided consistent and reliable results for imaging of the back of the eye where a good clear picture is necessary to diagnose retinal problems and intraocular tumors. The system also includes a fixed unit and a portable unit which adds flexibility and efficiency. Other systems do not provide this analysis capability to the extent of the VuMax HD Ophthalmic Ultrasound System.

4. Description of market research conducted and results or statement why it was not conducted:

A VIP search was conducted but failed to identify vendors capable of providing the requirement.

A search in ECAT was performed for this device but no results were found. Wholesale supply sources, such as Federal supply schedules were reviewed utilizing the GSA eLibrary and GSA Advantage website. Research showed there are no products that matched the term “VuMax HD Ophthalmic Ultrasound System.” Sonomed, Inc. (verified Small Business), is the sole manufacturer of the VuMax HD Ophthalmic Ultrasound System. Lombart Brothers, Inc. DBA Lombart Instrument (verified Small Business) is a verified authorized distributor of Sonomed equipment per email received on August 27, 2020 from Sonomed, Inc.

Sonomed, Inc. dba SonomedEscalon provided a “Letter of Manufacturer Attestation” dated September 1, 2020 stating that they are the sole manufacturer of the VuMax HD Ophthalmic Ultrasound System.

5. Requirements Certification: I certify that the requirement outlined in this justification is a Bona

Fide Need of the Department of Veterans Affairs and that the supporting data under my cognizance, which are included in the justification, are accurate and complete to the best of my knowledge and belief.
