About this file
This document is a Questions and Answers (Q&A) addendum for Request for Quote (RFQ) 36C25625Q1439 for Mattresses and Pumps. The Q&A addresses four key clarification points regarding product specifications and quantities. Specifically, the document discusses corrections to item numbers, concerns about weight capacity and surface dimensions for hospital and behavioral health platform bed mattresses, and discrepancies in the quantity breakdown between the price cost schedule and statement of work. The RFQ seeks 116 mattress surfaces (76 Low-Air-Loss with Microclimate Control and 40 Foam Surface Mattresses) and corresponding 116 pumps. The government indicates that revised CLINs Schedule, Statement of Work, and Price/Cost schedule have been attached to address the vendors' questions and provide updated specifications.

RFQ 36C25625Q1439 – Mattresses/Pumps
Questions and Answers
1. 7777-V803582DBH is missed typed and the correct item number is 7770-
V803582DBH
Please see attached revised CLINs Schedule as any model numbers have been removed. Quotes should be based on the specifications listed in the revised Statement of Work also attached.
2. You outline a weight capacity of up to 1,000 lbs for the hospital mattress, but you are requesting the 7770-NPT403584; that surface is not rated for that weight, nor are the Umano ook snow bed frames. Can you please clarify?
A revised CLINs schedule and SOW has been attached with this amendment.
Please see question above that any model numbers have been removed.
3. You outline these specifications below for your behavioral health platform bed mattresses:
-Weight capacity up to 1000 lbs -Width: 40.5 inches -Height: 16 inches -Patient sleep surface (width x length): 40.5 x 84.5 inches Can you please clarify specifications for these surfaces. It would be highly unusual and potentially dangerous to have a surface with a 16” height. We are confused about the other requests as well, the dimensions and also the rating up to 1000 pounds. If you could please clarify.
Please see revised Statement of Work attached.
4. The price cost schedule (8.2) requests (116) 7770-NPT40354 surfaces and (116) 7770-CU400 pumps but then the statement of work separates those into quantities of 76 and 40: Low-Air-Loss (Microclimate Control) – 76 surfaces and Foam Surface Mattress – 40 Surfaces. And the 40 foams include bullet points for dynamic pressure redistribution. Can you please clarify the request and need?
Please see updated Price/Cost schedule and SOW attached.
