About this file
This document contains questions and answers regarding Solicitation Number 36C26223Q1648 for 6515--IV Start Kits issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 22. The solicitation requires offers for both IV Start Kits and Pre-Filled Saline Syringes packaged 100 per case. The IV Start Kit must include a specific connector and extension tubing to interface with medical equipment. Packaging in a Tyvek pouch is acceptable provided all components are compatible with existing hospital equipment. The response due date was extended to August 24, 2023 at 4:00 PM PST. For a service-disabled veteran-owned small business supplying the end products of a large business, a waiver has been granted for certification requirements under the nonmanufacturer rule, though other SBA compliance certifications would still apply.

QUESTIONS AND ANSWERS:
1. On the RFQ document under Product Information it states 2 separate products. The first being the IV Start Kit & the second being the Pre-Filled Saline Syringe. Is this bid all or nothing or can we submit a bid on just the IV Start Kit? No, both items need to be offered or the offer will be considered non-responsive. (Requirement cannot be split)
2. The last component listed on the IV Start Kit states 1 EACH (RYM-6000GS) CNCTR, IV, INVISION-PLUS, NEUTRAL. Can you please clarify what this is? I believe it may be a small extension set from B Braun but need to make sure. If so, is this component absolutely needed? The last component is a connector and extension tubing to connect to the IV pump. Yes, it is absolutely required, ICU and ER require it not to be swapped out or exchanged because of medical staff current patient care practice.
3. We pack our standard off the shelf IV Start Kits 50 per case. You mentioned that you believed the current Medline pack is 100 per case. Please confirm for me. Yes, it has been confirmed that the current and requested packaging is 100 per case.
4. Will you accept a bid for only the IV Start Kit? No-bidding the Pre-Filled Saline Swab-flush w/Swab-cap? No, both items need to be offered or the offer will be considered non-responsive. (Requirement cannot be split)
5. Will you accept a kit in a header bag/Tyvek pouch vs a Form-Fill-Seal pouch? Tyvek bag is what we use primarily, and every component must be the same since it must be compatible with our current equipment in the OR and the Emergency Room.
6. Will you extend the due date in order to review answers to questions to prepare an accurate response? Yes, the response due date is now extended to 08-24-2023 @4:00 PM PST.
7. The RFQ includes VA NOTICE OF LIMITATIONS ON SUBCONTRACTING --CERTIFICATE OF COMPLIANCE FOR SUPPLIES AND PRODUCTS. ________is a Service-Disabled Veteran-Owned Small Business who will be supplying the requested end-item of a large business manufacturer and cannot certify the following:
Can you please provide guidance on how we can proceed? In this case, the nonmanufacturer rule waiver is granted and the certificate of compliance is tailored for that purpose. As the mentioned section states “unless a waiver is granted”.
However, a waiver has no effect on the SDVOSB requirements to certify on the other SBA Acts such as the Buy American Act or The Trade Agreements Act. Therefore, checking this box refers to your compliance with the other SBA Acts mentioned in the last sentence of that section (ii) and is not concerning if you will supply the product of a small business manufacturer.
