About this file

This is a Quality Assurance Surveillance Program document that establishes performance monitoring and oversight procedures for the de-installation and removal of GE INNOVA 4100-IQ imaging systems from the George E. Wahlen Veterans Affairs Medical Center in Salt Lake City, Utah. The program outlines surveillance mechanisms to ensure the contractor performs all labor, materials, equipment, and transportation necessary for complete removal and disposal of two imaging systems and associated peripheral subcomponents from the Radiology department in compliance with Original Equipment Manufacturer specifications and all applicable VA, federal, state, and local requirements.

The Quality Assurance Surveillance Program sets standards for contractor performance monitoring, including inspection protocols, documentation requirements, and compliance verification to ensure work meets contract specifications. The surveillance procedures apply throughout the removal project scope, with specific focus on proper de-installation methodology, safe handling and transportation of medical imaging equipment, appropriate disposal practices, and adherence to regulatory requirements. This program functions as a quality control mechanism administered by the Department of Veterans Affairs Veterans Health Administration to verify that the contractor fulfills all contractual obligations related to the removal of the medical imaging equipment from the VA facility.
