About this file
This document is a Quality Assurance Surveillance Plan (QASP) for switchboard services at the Hudson Valley VA Medical Center. The plan outlines performance standards and surveillance methods for evaluating contractor performance, with a 97% acceptable quality level across five key task areas: staffing coverage, call answering, telephone courtesy, call handling, and call screening. Surveillance methods include direct observation, periodic inspection, and validated customer complaints, with annual performance ratings ranging from Unsatisfactory to Excellent.
The document specifies detailed performance indicators for switchboard operators, including providing appropriate coverage during day shifts and emergencies, answering calls within five rings, maintaining telephone courtesy, protecting patient privacy, handling calls professionally, and ensuring operators do not misrepresent themselves as VA employees. The government will use past performance and potential non-payment as incentives for meeting or exceeding performance standards, with the Contracting Officer's Representative (COR) responsible for monitoring and evaluating the contractor's performance through various surveillance techniques.
