About this file

This is a Quality Assurance Surveillance Plan (QASP) for a radiological services contract. The QASP establishes eight performance objectives with associated performance measures and acceptable quality levels to monitor contractor compliance throughout the contract period. The Government reserves the right to modify the QASP unilaterally at any time.

The performance objectives require: (1) zero tolerance for late reports on interpretations and film returns; (2) zero tolerance for radiologist non-availability; (3) zero breaches of patient privacy and confidentiality; (4) mandatory maintenance of appropriate Federal and state licensure and certification prior to performance; (5) zero tolerance violations for documentation meeting accuracy, content, and signature standards; (6) 95% compliance for timely patient records completion within 48 hours of exam registration, with stat reports faxed within 45 minutes and critical/emergent results called to clinicians with documented proof; (7) zero breaches of HIPAA requirements; and (8) 100% adequate coverage with zero tolerance for violations. The Government will conduct periodic evaluations through appointed representatives who may increase inspections for repeated failures or customer complaints, or decrease inspections based on strong performance. VAPHS Quality Assurance Personnel will validate complaints and report to the Contracting Officer for enforcement action. Non-conforming services may be required to be re-performed at no additional cost, or the contract may be terminated if defects cannot be corrected through re-performance.

Likewise, the Government may decrease the number of quality assurance inspections if performance dictates.

VAPHS Quality Assurance Personnel will validate any complaints and report to the Government Contracting Officer who will then take necessary action to enforce contract compliance.

(b) If any of the services do not conform to contract requirements, the Government may require the Contractor to perform the services again in conformity with contract requirements, at no increase in contract amount. When the defects in services cannot be corrected by re-performance, the Government may result in contract termination.
