About this file
This is a Quality Assurance Surveillance Plan (QASP) for a phlebotomy services contract at the Michael E. DeBakey VA Medical Center in Houston, Texas, with a performance period from November 1, 2026 to October 31, 2031.
The contract requires the contractor to provide qualified phlebotomists to perform blood draw services for both inpatient and outpatient populations, including geriatric, spinal cord, and cancer patients. Phlebotomists must possess a minimum of two years of experience, including inpatient collection, outpatient experience, and proficiency with difficult venous access cases. Each phlebotomist must be capable of independently handling 40 to 80 patient draws per day and managing at least 20 percent of difficult sticks. Additional responsibilities include specimen accessioning, centrifugation, serum separation, test order entry, telephone management of test result requests, and rejection of compromised specimens according to protocol. The Government will monitor contractor performance against five primary performance measures: (1) Personnel availability and proper location coverage at 100 percent Acceptable Quality Level (AQL), measured through random inspection and time/attendance sheets; (2) Shift coverage for absences at 100 percent AQL; (3) Effective and courteous services with zero valid complaints at 100 percent AQL, validated through user/customer complaint tracking; (4) Proper patient identification using two approved identifiers and timely patient safety incident reporting within 24 hours at 100 percent AQL, verified through direct observation; and (5) Completion of mandatory privacy, confidentiality, and HIPAA training with zero privacy breaches at 100 percent AQL, documented through contractor-provided training evidence. The Contracting Officer and Contracting Officer's Representative will conduct quarterly performance assessment meetings, with performance ratings designated as Exceeds (2-5 percent above acceptable level), Meets (at acceptable level), or Does Not Meet (5 percent or more below acceptable level). Non-compliant performance will result in a Contract Discrepancy Report requiring a corrective action plan from the contractor.
