About this file
This Quality Assurance Surveillance Plan (QASP) outlines performance standards and monitoring methods for a HUD-VASH Housing Placement and Case Management Services contract with the Veterans Health Administration. The document establishes detailed performance requirements for a contractor providing housing and case management services to Veterans, with specific standards covering staffing, documentation, case management visits, housing placement, and discharge procedures.
Key surveillance methods include direct observation, periodic inspections, progress meetings, and customer feedback. The government will evaluate contractor performance monthly and quarterly, using an Acceptable Quality Level (AQL) for each performance requirement. Performance standards include maintaining up-to-date staff competency folders, documenting veteran interactions within 48 hours, completing clinical reminders, conducting appropriate case management visits, assisting veterans with housing placement, and minimizing negative program discharges. The Contracting Officer's Representative (COR) will conduct performance assessments and can issue Contract Discrepancy Reports for unacceptable performance.

This QASP is based on the premise, the Government desires to maintain a quality standard in providing primary care/mental health services to its patients. The resulting contract is considered the best means of achieving that objective.

• How monitoring shall take place.
• Who shall conduct the monitoring.
• How monitoring efforts and results shall be documented.
