QUALITY ASSURANCE SURVEILLANCE PLAN
FOR
Fire Extinguisher Maintenance Services Contract XXXXXXXXX
INTRODUCTION
This Quality Assurance Surveillance Plan (QASP) has been developed to evaluate contractor actions while implementing the SOW. It is designed to provide an effective surveillance method of monitoring contractor performance for each listed objective on the Services Summary in the maintenance contract.
The QASP provides a systematic method to evaluate the services the contractor is required to furnish.
This QASP is based on the premise the government desires to maintain a quality standard in operating, maintaining, and repairing facilities and that a service contract to provide the service is the best means of achieving that objective.
The contractor, and not the government, is responsible for management and quality control actions to meet the terms of the contract. The role of the government is quality assurance to ensure contract standards are achieved.
In this contract the quality control program is the driver for product quality. The contractor is required to develop a comprehensive program of inspections and monitoring actions. The first major step to ensuring a “self-correcting” contract is to ensure that the quality control program approved at the beginning of the contract provides the measures needed to lead the contractor to success.
Once the quality control program is approved, careful application of the process and standards presented in the remainder of this document will ensure a robust quality assurance program.

SURVEILLANCE STANDARDS AND PROCEDURES
Performance Standard Performance Threshold Perform Corrective Maintenance on fire Extinguishers to include proper labeling recharge. All work is completed according to frequency recommended by the manufacturer, NFPA 10 and T.O requirements.
100% of handheld portable fire extinguishers. Exception, No more than 10% of 150 lb Halon 1211 flight line extinguishers shall be out of service at any time
Perform Hydrostatic Testing on fire extinguishers. Fire Extinguishers shall be tested in accordance with NFPA 10, T.O, requirements and or manufacturer recommendations
100% compliance
Inspections and maintenance shall be logged on Daily Activity Forms
100% of data to be accurate and current at all times.
Maintain government provided equipment in good repair and working order.
100% compliance, with the exception of equipment out of service for repair or maintenance.
The contractor shall provide periodic updates on when repair or maintenance will be completed.
SURVEILLANCE: The government Contracting Officer Representative (COR) will observe the fire extinguisher maintenance technician in the setting of the Fire extinguisher shop located in Building 288 to ensure the requirements of SOW paragraphs 1.1 through 1.5. And paragraph 2 is being met. The COR will observe shop operations at least monthly to ensure they meet the requirements of SOW paragraph 1.2. The COR will evaluate Daily Activity Forms to ensure data is current at all times in accordance with SOW paragraph 1.3.
PROCEDURES: The Government COR will schedule inspections at least monthly. If Government COR discovers a defect(s), the Government COR will immediately notify the QCI for re-performance of the service to ensure compliance. However, as long the above standards are met. The contractor will be in compliance with the contract. If the standards are not met or if the QCI refuses to correct an identified defect, the Government QA will notify the contracting officer for appropriate action in accordance with the appropriate inspection of services clause.
